摘要
目的探讨哈萨克族(哈族)和汉族重度子痫前期(severe preeclampsia,S-PE)患者发病特点是否存在差异。方法将2012年1月-2013年1月新疆医科大学第一附属医院产科分娩的汉族与哈萨克族重度子痫前期患者按民族分为两组,比较两民族发病一般情况、合并症与并发症发生率是否存在差别。结果哈族患者发病年龄小于汉族患者(P<0.05),哈族患者发病孕周与终止妊娠孕周均早于汉族(P<0.05),哈族患者体质量指数高于汉族(P<0.05);两民族产次均以初产为主,差异无统计学意义(P>0.05);哈族患者合并妊娠期糖尿病、重度贫血、心衰、胎盘早剥以及胎儿生长受限和胎儿窘迫、死胎、肾功能损害发生率高于汉族(P<0.05);子痫、肝功能损害、HELLP综合征的发生率汉族患者高于哈族(P<0.05);两组脑血管意外、凝血功能障碍、肺栓塞、视网膜脱落、胎儿畸形、下肢静脉血栓形成和产后出血的发生率比较差异无统计学意义(P>0.05)。结论两民族重度子痫前期发病特点存在差异,环境和遗传均可能为其影响因素。
Objective To explore whether there are differences between Kazak and Han in clinical character of severe preeclampsia in Xinjiang. Methods A prospective study was conducted about general situation and complications of Han and Kazak severe preeclampsia patient in the First Affiliated Hospital of Xinjiang Medical University from January 2012 to January 2013. Results Kazak patients were younger than Han patients （P 〈0.05）, the onset and termination gestational age of Kazak patients were earlier than Han pa tients （P 〈0.05）, the body mass index of Kazak patients were higher than Han patients （P 〈0.05）. The parity was not statistics different between two groups （P〉0.05）. The incidence of gestational diabetes, severe anemia, cardiac failure, placental abruption, fetal distress and fetal growth restriction, stillbirth, renal damage in Kazak patients were higher than Han patients （P〈0.05）. The incidence of eclampsia, liv er damage, HELLP syndrome in Han patients was higher than Kazak patients （P〈0.05）, cerebrovascular accident, clotting disorders, pulmonary embolism, retinal detachment, maternal mortality, neonatal death, fetal abnormalities, and venous thrombosis of lower limb and the incidence of postpartum hemor rhage in the two groups were not statistically different （P〉0.05）. Conclusion There are difference of clinical characters of severe preeclampsia between Kazak and Han patients, which may be influenced by the environmental and genetic factors.
