摘要
目的探讨系统性红斑狼疮(systemic lupus erythematosus,SLE)及其合并自身免疫性甲状腺疾病(autoimmune thyroidism disease,AITD)患者的甲状腺功能。方法检测并分析比较72例SLE患者(SLE组)与80例健康体检者(健康对照组)甲状腺激素水平及合并AITD情况;对SLE合并AITD患者分别行抗双链DNA抗体(Anti-dsDNA)阳性、Anti-dsDNA及抗核小体抗体(ANuA)双阳性分组对比,并进行Anti-dsDNA定量与甲状腺功能和甲状腺自身抗体的相关性分析。结果①SLE患者FT3、FT4、TT3、TT4水平明显低于健康体检人群,AITD发病率、甲状腺自身抗体阳性率和促甲状腺素(TSH)明显高于健康体检人群,差异均有统计学意义(P<0.05)。②中高度活动SLE患者FT3、FT4、TT3、TT4明显低于轻度活动患者,TSH明显高于轻度活动患者,差异均有统计学意义(P<0.05)。③49例合并AITD的SLE患者中,Anti-dsDNA阳性(30例)与阴性(19例)、Anti-dsDNA及ANuA双阳性(24例)与非双抗体阳性(25例)的甲状腺素水平差异均无统计学意义(P>0.05)。④Anti-dsDNA定量与FT3、FT4、TT3、TT4、TSH、Anti-TPO、Anti-TG均无相关性(P>0.05)。结论 SLE患者易并发AITD,特别是甲状腺功能减退。甲状腺功能及甲状腺自身抗体与Anti-dsDNA、ANuA以及Anti-dsDNA定量无相关性。
Objective To study the thyroid antibodies and comorbidity of systemic lupus erythematosus(SLE)and autoimmune thyroid diseases,and analyze the relationship between autoimmune thyroid disease (AITD)and anti-double-stranded DNA antibody (Anti-dsDNA). Methods We Analyzed the level of tiayroid hormone and accompanied AITD in 72 SLE patients and 80 healthy vol- unteers. The SLE with AITD patients were separated into 2 groups, Anti-dsDNA positive group and Anti-dsDNA, anti-nucleosome anti- body( ANuA)double positive group, and group comparisons were made. We also performed a quantitative Anti-dsDNA determination and correlation detection between thyroid function and thyroid autoantibodies. Results The SLE patients has a much higher AITD prev- alence than that of the control group,and the TI3 ,FT3 ,FT4 and TT4 levels were significantly lower than those of the control group(P 〈 0.05 ), the TSH level was significantly higher than those of the control group( P 〈 0. 05 ). There is no significant difference in the TT3, FF3, FT4, TT4 and TSH levels between Anti-dsDNA positive, Anti-dsDNA, ANuA double positive patients and control group. There is no correlation between the quantitation of anti-dsDNA antibody and thyroid function together with thyroid autoantibodies( P 〉 0. 05 ). Conclusions Patients with SLE have a higher prevalence rate of autoimmune thyroid diseases, especially hypothyroidism, which is re- lated with active SLE. The thyroid function, thyroid autoantibodies, Anti-dsDNA, ANuA antibody and quantitation of anti-double-stran- ded DNA have no intereorrelation.
