生物蛋白海绵在额叶癫痫手术中的应用

摘要 目的前瞻性分析生物蛋白海绵和胶原海绵在额叶癫痫患者术中应用的有效性和安全性,探讨在额叶癫痫中应用生物蛋白海绵的价值。方法行前额叶切除术的的癫痫患者中,应用生物蛋白海绵止血(生物海绵组)和胶质蛋白海绵止血(胶原海绵组)的各28例。观察术后24 h头皮下引流管引流量、术后3d体温、术后第3 d腰椎穿刺时红细胞数和白细胞数。结果生物蛋白海绵组术后24h伤口引流量、3 d脑脊液中红细胞数明显低于胶原海绵组。生物蛋白海绵组出现脑室穿通比率显著低于胶质海绵组。两组术后第3 d晨起体温和脑脊液中白细胞数无统计学差异。结论生物蛋白海绵可以减少额叶癫痫术后脑组织创面的渗血,同时可能可以减少额叶癫痫术后脑室穿通的机率。 Objective To compare the efficiency and safety of biologic protein sponge with collagen sponge in surgery for patients with frontal lobe epilepsy, and explore the value of biologic protein sponge in epileptic surgery. Methods This cohort included 28 patients with frontal lobe epilepsy in biologic sponge group, who underwent anterior frontal lobectomy and used biologic protein sponge as hemostasis material in operation in our epileptic center. And control group, collagen sponge group, included another 28 cases, which underwent anterior frontal lobectomy and used collagen protein sponge as hemostasis material. The volumes of incision draining in 24 hours after operation, body temperature at morning, and numbers of red blood cells and white blood cells in cerebral spine fluid in third day after surgery were analyzed after 15 months treatment. Results Volumes of incision draining in 24 hours after operation in biologic sponge group was significantly lower than that in collagen sponge group. Number of red blood cells in cerebral spine fluid in the third day after surgery in biologic sponge group was also significantly lower than that in collagen sponge group; significant difference was found in percentage of porencephaly in patients with opening the lateral ventricle between two groups. Significant difference was not found in body temperature and number of white blood cells in cerebral spine fluid in 3 days after operation. Conclusion Biologic sponge can reduce bleeding of brain incision, and promote healing of dura, and decline the possibility of porencephaly after opening of lateral ventricle during anterior frontal lobectomy.
