摘要 目的调查胃癌患者术后早期的饮食及营养状况,为l临床营养护理提供参考。方法对70例行胃癌根治术的患者在人院时、出院时及术后4~6周时进行调查,评估其饮食、营养状况,调查患者接受饮食指导及在饮食恢复过程中遇到的问题。结果胃癌患者术后的饮食性质发生改变;入院时、出院时及术后4~6周时患者的每日进食量较未生病时减少。出院时患者的体质量、体质指数、上臂围、上臂肌围、白蛋白均低于人院时(P〈0.05);术后4~6周时患者的体质量、体质指数、三头肌皮褶厚度、上臂围、白蛋白均低于出院时(P〈0.05);术后4—6周时患者的体质量、体质指数、三头肌皮褶厚度、上臂围、上臂肌围均低于入院时(P〈0.05)。但发现患者得到的指导存在不足。结论胃癌患者术后的进食量不足,营养状况持续恶化,建议完善饮食指导的内容与方法,必要时为患者提供院外的饮食指导。 Objective To investigate the diet and nutritional status of gastric cancer patients during the early postoperative period after radical gastrecto- my, so as to provide references for clinical nutritional nursing. Methods Seventy gastric cancer patients undergoing radical gastrectomy were recruited and inves- tigated about their diet and nutritional status at admission, discharge and 4 to 6 weeks after surgery. Dietary guidance received by patients at discharge and prob- lems encountered in diet recovery process were also recorded. Results Obvious diet changes were observed among gastric cancer patients after surgery. Daily food intake decreased at admission, discharge and 4 to 6 weeks after surgery, more decreased after surgery. The patients'body weigh (BW) , body mass index (BMI) , arm circumference (AC) , arm muscle circumference ( AMC), albumin (ALB) were significantly lower than what were at admission ( P 〈 0.05 ). The patients" BW, BMI, triceps skin fold thickness (TSF) , AC and albumin 4 to 6 weeks after surgery were lower than what were at discharge ( P 〈 0.05 ). The patients" BW, BMI,TSF,AC and AMC 4 to 6 weeks after surgery were lower than what were at admission (P 〈 0.05 ). The instructions patients got were insufficient. Conclusion The postoperative intake of gastric cancer patients is insufficient, and their nutritional status continues deteriorating. It is suggested to perfect the contents and methods of dietary guidance and provide out - of - hospital dietary guidance when necessary.
