摘要 目的 分析住院患者下呼吸道感染非发酵菌的临床分布及耐药现状.方法 对榆林市第一医院2012年住院患者下呼吸道感染标本分离的4种非发酵菌进行临床分布及耐药性分析.结果 在下呼吸道感染标本中共分离出非发酵菌1 012株,其中铜绿假单胞菌512株(50.6%),鲍曼不动杆菌256株(25.3%),嗜麦芽窄食单胞菌153株(15.1%),洋葱伯克霍尔德菌49株(4.8%),其他非发酵菌42株(4.2%).4种主要非发酵菌普遍呈多重耐药趋势,由于不同的非发酵菌耐药谱各不相同,嗜麦芽窄食单胞菌对碳氢霉烯类天然耐药,所以细菌鉴定和药敏试验十分重要.结论 下呼吸道标本分离的非发酵菌患者主要分布在重症监护室、呼吸内科、心血管内科等,下呼吸道感染非发酵菌的耐药性比较严重,临床应依据细菌学药敏报告合理使用抗菌素,以提高疗效. Objective To analyze of hospitalized patients with lower respiratory tract infections non-fermenters Clinical Distri- bution and drug resistance. Methods Analyzed the clinical distribution and drug resistance of four kinds of non-fermented bacilli separared from the specimens of the hospitalized patients with lower respiratory tract infection in the First Hospital of Yulin City in 2012. Results Lower respiratory tract infections were separated non-fermenting bacteria 1 012,of which P. aeruginosa 512 (50. 6%) ,Acinetobacterbaumannii 256(25.3~) ,Stenotrophomonas narrow food Aeromonas 153(15.1 ~) Burkholderia cepacia 49(4. 8Y0) ,and other non-fermenters 4Z(4.2~). The four main kinds of non-fermenting bacteria were multi-drug resistant universahrend,due to the different non-fermenting bacteria resistance patterns vary Stenotrophomonas narrow food Aeromonas natural resistance against hydrocarbon enzyme vinyl, so very bacterial identification and susceptibili- ty testing important. Conclusion Lower respiratory tract specimens from patients with non-fermenting bacteria, mainly in the intensive care unit, respiratory medicine, cardiology, etc. , lower respiratory tract infections resistant non-fermenting bac- teria more serious clinical reports should be based on the rational use of antimicrobial susceptibility bacteriology to improve efficacy.
