摘要 目的:探讨早期胸部物理治疗在全胸腔镜下心脏手术患者术后肺部护理中的效果。方法:对150例全胸腔镜下行心脏手术患者采用自身对照方法,术后呼吸机辅助呼吸加用PEEP(呼气末正压),在拔除气管插管前充分吸痰、膨肺,拔除气管插管后行雾化吸入,血流动力学稳定后采取半坐卧位,指导患者有效咳嗽、咳痰,进行拍背、震颤、体位引流等治疗。结果:治疗后较治疗前血气分析结果显示动脉氧分压明显升高,二氧化碳分压明显下降(P<0.05),X线胸片显示肺部无明显炎症表现。结论:全胸腔镜下心脏手术后患者早期行胸部物理治疗,对预防肺不张、肺部感染、低氧血症等肺部并发症具有重要的临床应用价值。 Objective:To explore the effect of the early chest physiotherapy on postoperative patients with totally thoracoscopic cardiac operation. Methods :The control method was adopted in this group (150 cases ). Ventilator- assisted breathing plus PEEP was used. Spu- tum aspiration and lung inflation was fully done before extubation and such treatment as aerosol inhalation, expectoration and postural drainage were given to the patients after extubation. Results :The partial pressure of oxygen in arterial blood significantly increased and par- tial pressure of carbon dioxide significantly decreased after treatment compared with those before treatment ( P 〈 0.05 ). The chest X - ray showed no obvious inflammation in the lungs. Conclusion :The early chest physiotherapy for postoperative patients with totally thoraeoscopic cardiac operation has an important clinical value in the prevention of pulmonary complications like pulmonary atelectasis, pulmonary infec- tion, hypoxemia, etc.
