摘要
目的探讨二维及彩色多普勒超声鉴别诊断卵巢良恶性肿瘤的价值。方法收集该院自2010年1月~2012年12月在该科行超声检查并经手术和病理证实的卵巢肿瘤患者127例的临床资料。其中,卵巢良性肿瘤75例,恶性肿瘤52例,二维超声常规检查附件区肿物的大小,有无包膜,内部结构及有无腹水。引入彩色多普勒仔细观察病灶周边及内部血流情况。结果二维超声观察卵巢良恶性肿瘤特点:良性肿瘤75例,63个肿瘤有包膜,57个肿瘤包膜完整规则,84个肿瘤以囊性为主;恶性肿瘤52例,仅9个有包膜,其中7个肿瘤包膜不完整且形态不规则,42个为实性或囊实混合性,两组间的形态表现差异有显著性(P〈0.01)。卵巢恶性肿瘤60例中有32例检出腹水,占53%(32/60),良性肿瘤中只有3例检查出腹水,两者比较差异有显著性(P〈0.01)。卵巢恶性肿瘤峰值流速(21.54±7.93)cm/s,平均流速(13.56±4.99)cm/s,PI值(0.84±0.32),RI值(0.39±0.14),卵巢良性肿瘤峰值流速(13.18±7.42)cm/s,平均流速(8.27±5.29)cm/s,PI值(1.37±0.35),RI值(067±0.17),两者比较差异有显著性(P〈0.01)。结论对卵巢肿瘤良恶性的超声鉴别诊断,二维超声观察肿瘤的形态和内部结构以及对腹水的探查,彩色多普勒观察肿瘤的血流分布情况,二者联合应用诊断准确率明显提高。
[ Objective ] To discuss the clinical value of two-dimension ultrasound and color Doppler flow image in diagnosis of ovarian neoplasms. [ Method ] 127 cases of ovarian neoplasms proved by pathology and operation were examined by ultrasound from Jan.2010 to Oct.2012, among them, benign tumor was 75 cases, malignant tumor was 52 cases, the tumor in addexa was examined routinely by two-dimension ultrasound: including size, figure, amicu- lure, internal structure and ascites. The focus was watched carefully to see blood flow inside and outside. The char- acteristics of ovarian neoplasms were observed by two-dimension ultrasound. [ Results ] There were 75 cases of be- nign tumor, 63 tumors had amiculums, 57 tumors had complete and regular amiculums, 49 tumors showed cystic; while there were 52 cases of malignant tumor, only 9 of them had amiculums, among them 7 had complete amicu- lums, 42 tumors showed solid tumor with cystic component inside, they two had apparent statistical significance (P 〈 0.01). Ascites can be detected in 41 cases among 52 patients with malignant ovarian tumor, account for 78.85%, on- ly 3 cases with ascites be found in benign tumors, there was a significant difference between two groups (P 〈0.01); There were 75 cases of benign tumor: Peak systolic velocity (13.18±7.42) cm/s, average maximum velocity (8.27± 5.29) cm/s, PI (1.37±0.35) and RI (0.67±0.17); We got the ovarian carcinoma blow flow parameter as follows: Peak systolic velocity (21.54±7.93) cm/s, average maximum velocity (13.56±4.99) cm/s, PI (0.84±0.32) and RI (0.39± 0.14). [ Conclusion ] Firstly, we should use two-dimension ultrasound to detect the tumor's figure and internal strnc-tures, so did the observation of ascites, then observe the blood flow of tumor. The accuracy of diagnosing ovarian tu- mors has been enhanced, and also have higher sensitivity and specificity.
