摘要
目的探讨肝脏孤立性坏死结节(SNN)的MRI平扫和动态增强特征及病理表现,以提高对SNN的诊断。资料与方法回顾性分析经病理证实的10例SNN患者的MRI征象,对病灶数目、形状、大小、部位、边界,以及MRI平扫和增强后信号、强化方式进行评价,并与病理结果进行比较。结果共发现10个病灶,平扫T1WI示7个病灶呈稍低信号,3个呈等信号;T2WI示2个病灶呈稍高信号,8个呈等信号,其中2个等信号及1个稍高信号病灶内见点状或小斑片状高信号;DWI示5个呈病灶呈等信号,5个呈稍高信号。10个病灶增强扫描后内部均未见强化,在门静脉及延迟期呈明显低信号,边界及形态显示清楚,边缘强化。病理表现:肝脏SNN中央为凝固性坏死物质,外周的薄层纤维包膜中有淋巴细胞、浆细胞、嗜酸性细胞及少数中性粒细胞浸润,结节周围的肝细胞基本正常。结论 MRI平扫和动态增强能较好地反映SNN的影像学特点,有助于SNN的诊断。
Purpose To explore the pathological findings and MR1 tbatures of hepatic solitary necrotic nodule (SNN) in non-enhanced and dynamic contrast-enhanced MRI scan, and to improve the awareness for SNN diagnosis. Materials and Methods MRI features of l0 patients with pathologically confirmed SNN were retrospectively analyzed, the amount, shape, size, location, boundary, signal intensity and enhancement pattern in unenhanced and enhanced MRI of the hepatic lesions were evaluated and compared with the pathological results. Results A total of 10 lesions were detected, unenhanced T IWI showed seven lesions with slightly lower signal and three lesions with equal signal: T2WI showed two lesions with slightly higher signal, eight with equal signal, including punctate or small patchy high signal lesions inside the two equal signal and one slightly higher signal lesions; DWI images showed five lesions with equal signal and five with slightly higher signal. All the 10 lesions showed no contrast enhancement in the enhanced scan and appeared as low signal with clear boundaries and morphology with edge enhancement in portal venous phase and delayed phase, Pathology manifestation: coagulation necrosis substances located in the central part of hepatic SNN, surrounded by peripheral fibrous capsule which was filled with lymphocytes, plasma cells and eosinophils, with minority neutrophil infiltration, hepatic cells around the nodule were basically normal. Conclusion Unenhanced and enhanced MRI can reflect SNN imaging features better, thus will help with the diagnosis of SNN.
