Disc—FX微创手术系统治疗椎间盘源性腰痛35例临床分析

Clinical analysis of the 3-in-1 technique of Disc-FX system for discogenic low back pain

摘要 目的分析采用Disc—FX微创手术系统下行髓核摘除、射频消融和纤维环成型的“三合一”方法治疗椎间盘源性腰痛的临床疗效。方法2011年2月至2012年5月,对35例椎间盘源性腰痛患者行Disc—FX通道系统辅助下的“三合一”治疗,其中男23例,女12例;年龄27~56岁,平均36.5岁。对比分析治疗前和术后1周、6个月及末次随访时VAS评分及Oswestry评分。结果手术时间25~45min,平均29min。术后随访6~18个月,平均14.8个月。术后未发现手术相关并发症。术后1周、6个月以及末次随访时VAS评分分别为:(0.954-0.58)分、(1.15±0.60)分和(1.05±0.66)分,与术前VAS评分(6.70±1.26)分相比,差异有统计学意义(P〈0.05);Oswestry评分分别为(8.3±5.7)分、(8.5±4.9)分、(8.6±4.5)分,与术前(19.9±6.8)分相比,评分结果比较差异均有统计学意义(P〈0.05)。有1例患者术后2个月时复发再次手术。结论Disc—FX系统辅助下的“三合一”技术治疗椎间盘源性腰痛近期疗效满意,尚需增加观察例数,其远期效果需进一步观察。 Objective To study the clinical efficacy of the 3-in-1 technique of Disc-FX system, including: discectomy, radiofrequency ablation and annuloplasty for discogenic low back pain. Methods From February 2011 to May 2012, 35 patients with the discogenic low back pain were treated discecto-my, radiofrequency ablation and annuloplasty by Disc-FX system, cluding 23 males and 12 females with an average age of 36. 5 years old ( range, 27-56 years old). The visual analogue scale (VAS) scoring and Oswestry scoring system were applied to evaluate the pain at preoperative, postoperative 1 week, 6 months, and the final follow-up. The Oswestry score was also evaluated. They were analyzed retrospec- tively. Results All the patients were followed up for average 14. 8 months (range from 6 to 18 months). The operative time averaged 29 min (range, 25 - 45 rain). No complication was found dur-ing the follow-up time. The VAS score of preoperative was (6.70 ± 1.26) scores, VAS score of pain at postoperative 1 week, 6 months,and final follow-up were (0.95 ± 0. 58)scores, (1.15 ± 0. 60)scores, ( 1.05 ± 0. 66) scores respectively. The postoperative VAS of pain decreased significantly compared with that of preoperative. The Oswestry score of preoperation was ( 19.9 ± 6. 8) scores, postoperative 1 week, 6 months, and final follow-up were (8.3 ± 5.7) scores, (8.5 ± 4.9) scores, (8.6 ± 4. 5) scores respec-tively. It was significantly compared with that of preoperative also. Conclusions Disc-FX system,which collected three surgical tec procedure, provides a discogenic low back pain. clinical outcomes during a que in new choice one for It give satisfactory short-term follow up,but for the long term clinical outcomes, it remains unknown.
