摘要 滑车上神经为三叉神经额神经的分支,经滑车上方从内眶出孔,在皱眉肌外侧和穿经皱眉肌后穿过额肌到达头皮,分布于额部皮肤,和眶上神经一起建立额部皮肤的浅表感觉。其支配额部皮肤范围较广,且有恒定的滑车上动脉穿支营养,对额部皮肤的血供和感觉维持具有重要意义。以其为蒂的额旁正中滑车上神经营养血管皮瓣具有以下优点(:1)手术在同一部位进行,一次完成,操作简单快捷(;2)不损伤主干血管,对供区血供破坏少;(3)皮瓣带有皮神经,在受区进行吻合,可恢复感觉功能;(4)设计灵活,可沿皮神经任意切取皮瓣,能满足大部鼻缺损的需要。在为接受鼻缺损修复术的患者重塑鼻部外形的同时,消除了传统皮瓣所引起的皮肤感觉缺失,皮瓣发红等诸多不良反应,是鼻缺损修复术中的一大亮点,值得引起相关学者的关注。 Supratrochlear nerve is the branch of frontal nerve of trigeminal, nerve, that is along above of the supratrochlear from the inside hole, orbiting in corrugator lateral and drawing-in corrugator go through the frontal muscle to scalp, distributes among forehead skin, with supraorbital nerve together to establish the sensation of forehead.It is a quota of skin with wider range, and a constant supratrochlear artery wear a nutrition, the frontal part of the skin to maintain blood supply and getting feel is of great significance.As the. speed of the frontal side on the middle of the supratrochlear nerve nutrition tube flap has the following advantages: ( 1 ) The operation is performed in the same position, once completed, the operation is simply and quickly; ( 2 ) No main blood vessels are to be damaged, blood supply of donor area has less damage; ( 3 ) Skin flap with cutaneous nerve, under area for anastomosis can recover feeling function; ( 4 ) Design freely along the skin nerve arbitrary chipped flap, which can meet the needs of most nose defect.To accept the nose defect prosthesis of patients to reshape the nasal shape, at the same time, it also can eliminate the feeling loss caused by traditional skin flap, flap anesthesia red and so on many adverse reactions, make nose defect prosthesis in a big window, and some scholars should pay attention to it.
