摘要
目的比较盐酸氯普鲁卡因和盐酸利多卡因在硬膜外麻醉中用于剖宫产手术的效果和安全性.方法将80例ASAⅠ-Ⅱ级行剖宫产手术产妇随机分成两组,试验组(3%氯普鲁卡因)和对照组(2%利多卡因),每组各40例.分别于腰12~3间隙行硬膜外穿刺置管,试验剂量为3mL,观察无毒性反应.于5,10rain各追加5mL药物,比较两组麻醉起效时间、痛觉消失时间、痛觉恢复时间、运动恢复时间,评价手术肌松效果,观察产妇有无不良反应及新生儿Apgar评分.结果试验组起效快,痛觉消失快,但与对照组比较无统计学意义;试验组痛觉及运动恢复较快,与对照组比较有统计学意义;两组问手术肌松效果均满意,产妇均无不良反应;新生儿Apgar评分比较无统计学意义.结论盐酸氯普鲁卡因可用于剖宫产手术麻醉,与盐酸利多卡因麻醉效果满意度相近,而且恢复更快、更安全.
Objective To compare the effect and safety of lidocaine hydrochloride and chloroprocaine hydrochloride in the epidural anesthesia for cesarean delivery. Method 80 Postpartum women who had received ASAⅠ-Ⅱ cesarean operation were randomly divided into two groups, experimental group (3% chlorprocaine hydrochloride) and control group (2% lidocaine hydrochloride), and there were 40 patients in each group. The epidural puncture tubes were placed between lumbar 2 and 3 ,the test dose was 3 mL, and no toxic reaction was found. 5 Minutes and 10 minutes later, additional 5mL of the drugs were given separately, and then, the onset time of anesthesia, analgesia time, pain recovery time, movement recovery time, muscle relaxation effect of the drugs were compared and evaluated. Meanwhile, adverse reactions of the drugs on the postpartum women were observed and neonatal Apgar scores were assessed. Results In the experimental group, a rapid onset time and pain sense disappearance time could be seen, but there was no significant difference compared with those in the control group. However, the pain recovery and movement recovery in the experimental group were faster than those in the control group significantly. In both groups, the muscle relaxation effect during the operation was satisfied no adverse reaction was seen in the postpartum women, and the difference in Apgar score of neonates was not significant. Conclusion Chloroprocaine hydrochloride can be used for the anesthesia for cesarean section operation,the anesthesia effect satisfaction should be similar to that of lidocaine hydrochloride,and it can make the patients recover faster from the anesthesia,indicating that it may be safer.
