InterTan髓内钉治疗不稳定型骨质疏松性股骨转子间骨折近期疗效 被引量:15

摘要 目的探讨股骨近端InterTan髓内钉治疗不稳定型髓内钉老年骨质疏松性股骨转子间骨折的近期临床疗效。方法对53例不稳定型的股骨转子间骨折使用InterTan髓内钉治疗。对术后并发症、负重行走时间、Harris髋关节评分、x线随访及骨折愈合时间等情况进行评估。结果53例均获随访,时间9~12(10.53±0.42)个月。术后Harris髋关节评分60~96(84.47±2.19)分,优良率79.25%;骨折愈合时间8~12(9.77±0.38)周。无股骨头颈部短缩、髋内翻、内植物周围骨折和内固定物失效、断裂发生。结论InterTan髓内钉治疗伴骨质疏松的不稳定型老年股骨转子间骨折并发症少,骨折愈合率高,近期临床疗效满意。 Objective To investigate the short-term clinical results of intramedullary nail of InterTan for unstable os- teoporotic femoral intertrochanteric fractures. Methods 53 patients with unstable osteoporotic femoral intertrochan- teric fracture were treated with InterTan. The complications, walk, Harris hip score, X-radiograph and the time of fracture healing were evaluated. Results The follow-up time was 9 - 12 ( 10. 53 ± 0.42 ) months. According to Harris hip score, the postoperation score was 60 -90 (84. 47 ± 2. 19) points, the excellent and good rate at the final follow- up was 79.25%. All fi'actures healed within 8 - 12 (9. 77 ±0. 38) weeks. No collapse of the femoral neck, varus malposition, femoral shaft fracture around the intramedullary nail, implants loosening and internal fixation failure were found. Conclusions The new intertrochanteric antegrade nail of InterTan is effective for treating unstable osteo- porotic femoral intertrochanteric fractures with good results, less complications, and high union rate.
