摘要
目的探讨儿童急性中耳炎的特点及治疗。方法回顾性分析35例急性中耳炎患儿临床资料,其中男21例(耳),女14例(耳),均以耳痛为第一主诉,10例伴鼻窦炎,20例伴上呼吸道感染,5例伴急性化脓性扁桃体炎(1例伴面瘫)。予头孢曲松纳静脉滴注,抗病毒治疗连续用7 d,红外线照射耳部治疗,1例面瘫加用地塞米松及神经营养药,短期使用鼻腔减充血剂,解热镇痛药。2 w后复查听力及耳内窥镜检查。结果 32例患儿听力恢复正常,症状消失;1个月后面瘫患儿治愈。2例遗留鼓膜穿孔,3个月后行鼓膜修补术。结论儿童急性中耳炎是儿童临床常见疾病,发病率高,进展快,应早期诊断、及时治疗,否则可导致严重的并发症或转为慢性中耳炎影响患儿听力。头孢曲松抗菌谱广,对儿童急性中耳炎常见致病菌敏感,治疗安全、有效,可作为一线用药。
Objective To study the treatment of acute otitis media in children and its characteristics. Methods A retrospective analysis was made in the clinic data of 35 children（35 ears）with acute otitis media including 21 male cases （ears）and 14 female cases（ears）. Otalgia was the first main complaint. 10 cases were accompanied with sinusitis, 20 ones with upper respiratory infection, and 5 ones with acute suppurative tonsillitis（ one case was accompanied with facial paralysis）. They received the intravenous drip with ceftriaxone, the antiviral treatment for 7 days, and infrared radiation for the aural region. The case with facial paralysis received dexamethasone and neurotrophic medicine and also received decongestant and antipyretic analgesic in short time. Two weeks later, they received the reexamination for hearing and the aural splanchnoscopic examination. Results Thirty two cases gained full recovery of hearing, and their symptoms disappeared. One month later, the case of facial palsy was cured. Two cases complicated with perforation of the ear drum were cured by tympanic membrane repairing after three months. Conclusion Acute otitis media is the common disease among children. It has high attack rate and fast advancement and should be diagnosed and treated as early as possible. Ceftriaxone has a broad antimicrobial spectrum and is sensitive to common pathogenic bacteria of acute otitis media in children. It can be used safely in children.
