摘要 目的探讨磁敏感成像(SWI)技术对于发现弥漫性轴索损伤(DAI)的出血性病灶的应用价值。方法对临床诊断为DAI的19患者,受伤两周内行头部磁共振常规序列及SWI序列扫描,将SWI序列与常规T2WI序列发现的病灶数目进行比较。并对SWI序列发现的病灶数与患者入院时的格拉斯哥昏迷评分(GCS)进行相关性分析。结果 SWI序列、T2WI序列、T1WI序列发现病灶数目分别为:538个、128个、101个。SWI序列对于发现DAI病灶的能力显著高于常规T2WI序列(Z=-3.764,P<0.01),并且DAI病灶的数目与患者入院时G C S评分呈高度的负相关(r=-0.746,P<0.01)。结论 SWI序列能较MRI常规序列更敏感的发现DAI出血性病灶,对临床医生准确评估DAI患者病情具有重要意义。 Objective To investigate the significance of Susceptibility Weighted Imaging (SWI) in diagnosis of hemorrhagic lesions associated with Diffuse axonal injury (DAI). Methods 19 acute or subacute DA1 patients are examined using SWI and conventional MRI sequence. To compare the sensitivity of SWI with T2Wl in detecting lesions, and correlate the number of lesions with initial GCS. Results The number of lesions detected was l01, 128 and 538 respectively on TIWI, T2WI and SWI. The number of lesions detected by SWI is statistical more than conventional sequence (Z=-3.764, P〈0.01 ). The number of lesions in SWI of the 19 patients is negatively correlated markedly with GCS(r=-0.746, P 〈0.01). Conclusion SWI can be more completely to display hemorrhagic lesions associated with DAI and be better estimated the condition of patients.
