摘要
目的 :研究再生障碍性贫血 (AA)患者外周及骨髓血IL 2、sIL 2R、IL 3、IL 6、IL 8、TNF α、SCF和IFN γ含量变化与骨髓造血功能的改变及临床转归的关系。方法 :采用夹心酶联免疫吸附法检测AA外周及骨髓血中细胞因子含量的动态变化。结果 :1.治疗前患者血浆及骨髓中IL 2、sIL 2R、IL 3、IL 6、IL 8、TNF α和IFN γ浓度与正常对照组均有不同程度的升高 ,以IL 2、IL 8、TNF α和IFN γ水平升高更明显 (P <0 .0 1) ,SCF则无改变 ;2 .急、慢性AA(sAA ,cAA)治疗前后细胞因子浓度的比较发现cAA血浆及骨髓IL 3高于sAA(P <0 .0 5 ) ,而IL 2、IL 8、TNF α和IFN γ浓度则以sAA升高明显 (P <0 .0 1) ;3.经治疗有效的患者几种因子均有明显的降低 (P <0 .0 5 ) ,尤以IL 2、IL 8、TNF α和IFN γ下降明显 (P <0 .0 1)。结论 :IL 2、IL 8、TNF α和IFN γ与AA的发生、发展有关。
Objective:To investigate the relationship between the levels of interleukin-2, 3, 6, 8, soluble interleukin -2 receptor (sIL-2R), steel factor(SCF) tumor necrosis factor-α(TNF-α)and interferon-γ (IFN-γ) in serum and bone marrow and hematopoiesis in patients with aplastic anemia(AA) and its implication on clinical outcome. Methods: Using enzyme-linked immunoadsorbent assay(ELISA), have studied pre- and post-treatment serum and bone marrow level of IL-2, IL-2R, IL-3, IL-6, IL-8, SCF, TNF-α and IFN-γ in 37 patients with AA. Results: In compared with controls, the pretreatment levels of IL-2, sIL-2R,IL-3, IL-6, IL-8 , TNF-α and IFN-γ in serum and bone marrow were slightly higher in patients with AA. But the levels of IL-2, IL-8 and IFN-γ were much higher in these patients. The level of SCF was unchanged. Serum and bone marrow IL-3 level were much higher in patients with chronic AA(P<0.05), otherwise higher level of IL-2, IL-8, TNF-α and IFN-γ levels was always seen in patients with serious AA(sAA, P<0.01). After successful treatment, the levels of cytokines detected in serum and bone marrow tended to return to normal level(P<0.05), and decrease of IL-2, IL-8, TNF-α and IFN-γ levels were much more significant(P<0.01). Conclusion: The result of the study on AA showed that interdependent role of IL-2, IL-8, TNF-α and IFN-γ on pathogenesis and developing of AA.
