摘要
目的:分析2型糖尿病下肢动脉硬化不同程度患者的骨密度变化,探讨2型糖尿病和骨质疏松之间的相关性。方法:本研究选取2型糖尿病患者108例,其中男56例,女52例,按下肢动脉血管病变严重程度分为无病变组、轻度病变组、中重度病变组,同时采用双能X线吸收仪测定正位腰椎、左侧股骨及左前臂骨密度变化,并结合患者的性别、年龄、体质指数、病程、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、极低密度脂蛋白胆固醇、空腹血糖、糖化血红蛋白(HbA1C)、尿白蛋白进行相关性分析。结果:①无病变组左股骨骨密度显著高于轻度、中重度病变组(均P<0.05),轻度病变组左股骨颈骨密度显著高于中重度病变组(P<0.05)。无病变组左前臂骨密度显著高于轻度、中重度病变组(均P<0.05),轻度病变组左前臂骨密度显著高于中重度病变组(P<0.05)。且糖尿病动脉硬化程度越重,骨密度下降越明显。②无病变组血总胆固醇显著低于轻度、中重度病变组(均P<0.05)。甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、空腹血糖、HbA1C、尿白蛋白:中重度病变组与无病变组及轻度病变2组均存在显著差异(P<0.05)。③随着BMI值升高,骨密度值也显著升高(P<0.05);随着病程增加、动脉硬化积分及HbA1C值升高,骨密度值显著降低(均P<0.05)。结论:2型糖尿病下肢血管病变与骨密度有一定相关性。有动脉硬化的患者也存在骨密度降低风险。
Objective :To discuss the relationship between osteoporosis and arteriosclerosis of lower-extremity by analyzing degrees of bone mineral density(BMD)in patients with type 2 diabetes combined with arteriosclerosis of lower extremity.Methods:One hundred and eight patients with type 2 diabetes including 56 male cases and 52 female cases.The arterial wall endometrial thickness ,luminal diameter,plaque and blood flow of lower extremity were dectected by ultrasound.And the patients were divided into 3 groups (non-arteriosclerosis group, mild group,moderate/severe group)according to the severity of lower-extremity artery disease.BMD of lumbar spine,left femur,left forearm were measured by dual cherty X-ray absorptiometry.All of the subjects determined clinical index and analyzed risk factors.Results:(1)In the non-arteriosclerosis group,the BMD of left femur,left forearm were both higher than the other 2 groups.The mild group of BMD in the left femur,left forearm were higher than the moderate/severe group.The more severe arteriosclerosis,the lower the BMD was. (2)In the non-arteriosclerosis group total cholesterol was lower than other two groups (P〈0.05).TG,HDL,LDL, BG,glycosylated hemoglobin (HbA1C),UA in moderate/severe group had significant difference with the other 2 groups(P〈0.05).With the increase of body mass index,the BMD was higher(P〈0.05).With the increase of diabetes duration,arteries atherosclerosis extent integral and HbA1C,decrease in BMD (P〈0.05).Conclusion:Lower extremity vascular disease of diabetes may related to BMD.Early determination of arteriosclerosis have risks of reduction of BMD.
