卡式微柱凝胶法与凝聚胺法在临床配血相容性中的应用 被引量:6

COMPARATIVE STUDY OF MICRO COLUMN GEL METHOD WITH CONDENSED AMINE METHOD IN THE CLINICAL BLOOD COMPATIBILITY

摘要 目的对微柱凝胶法与凝聚胺法二种方法在临床配血相容性的比对。方法采用微柱凝胶法与凝聚胺法同时对输血患者进行交叉配血和患者血清不规则抗体检测双盲平行试验,阳性结果利用传统手工抗人球法比较其准确性和灵敏度。结果在12 620例申请输血的患者中,微柱凝胶法交叉配血有主测和次测不相合24例,占0.19%,其中主测17例,次测6例,假阳性1例。检出血浆不规则抗体阳性17例,直接抗人球蛋白阳性6例,假阳性1例。凝聚胺法交叉配血主测和次测不相合15例,不相合率占0.12%,其中主测12例,次测3例,检出血浆不规则抗体12例,直接抗人球蛋白阳性3例。结论在输血工作中选择微柱凝胶法进行输血相容性检测更能对预防各类急慢溶血性输血反应,减少无效输血的发生。 Objective To compare microcolumn gel method with condensed amine method in the clinical blood compatibility. Methods Both micro column gel method and condensed amine method were used to cross matching the blood transfusion and test serum irregular antibody which was a double - blind parallel detection for patients with blood transfusion. The positive sample would compare the accuracy and sensitivity by using traditional manual anti- human immune globulin method. Results In 12 620 cases of application of transfusion patients, 24 cases were out of keeping with by micro column gel method cross matching, accounting for 0.19%, among them, 17 cases main measure, 6 cases secondary measure, 1 case false posi- tive. 17 cases of them were plasma irregular antibody positive, 6 cases direct against human ball white positive, 1 cases false positive. Condensed Amine method were out of keeping with on the main and the sec- ondary measurement cross matching main measurement the secondary test in 15 cases, did not agree rate 0. 12%, including 12 cases primary and 3 cases secondary measurement, detection of plasma irregular anti- body in 12 cases, direct anti - human immune globulin positive in 3 cases. Conclusion In blood transfusion, to choose the micro column gel method for blood transfusion compatibility testing is better able to prevent all kinds of quick / slow hemolytic transfusion reaction, to ensure that the occurrence of invalid transfusion.
