摘要
目的探讨挫伤性视神经病变患者图形视觉诱发电位(P—VEP)振幅下降百分比与客观判断单眼盲的关系。方法回顾性分析31例单眼挫伤性视神经病变病例,将患者按伤眼视力≥0.05与〈0.05分为两组,比较两组间伤眼较对侧健眼P—VEP振幅下降百分比的差异,通过ROC曲线寻找P—VEP振幅下降百分比在两组间的临界值。结果两组患者的P—VEP振幅下降百分比具有统计学显著差异(P〈0.05)。60′方格的P—VEP振幅下降百分比的受试者工作特征曲线(ROC)的曲线下面积(AUC)为0.804,敏感性与特异性最佳点为42.68%,30′方格的AUC为0.719,敏感性及特异性最佳点为67.4%。结论在发生挫伤性视神经病变时,当伤眼较对侧健眼P—VEP振幅下降百分比在60′方格及30′方格中分别达到42.68%与67.4%时,对客观判断单眼盲诊断具有参考价值。
Objective To investigate the predication of unilateral blindness due to contusive optic neuropathy from the percentage of decreased pattern reverse visual evoked potential (P -VEP) amplitude. Methods 31 cases of unilateral contusion optic neuropathy were analyzed retrospectively. They were divided into two groups according to the visual acuity of injuried eye ( ≥ 0. 05 and 〈 0.05 ). The percentage of decreased P- VEP amplitude was calculated from the difference between the injured eye and contralateral healthy eye. The difference of percentage of decreased P - VEP amplitude between the two groups was statistically analyzed. Receiver operating characteristic curve (ROC) was used to evaluate the cutoff value of two groups. Results The percentages of decreased P - VEP amplitude between two groups were significantly different ( P 〈 0.05 ). The area under curve (AUC) of ROC curve of the percentage of decreased P - VEP amplitude at 60' grid was 0.804, and the cutoff value was 42.68% ; meanwhile, the AUC at 30′ grid was 0. 719, and the cutoff value was 67.4%. Conclusion When the percentage of decreased P - VEP amplitude at 60' grid or 30' grid in unilateral contusion optic neuropathy is 42.68% or 67.4 % respectively, it is indicated the blindness of injured eye.
