摘要
目的:观察关节镜清理术联合活血消肿汤治疗膝关节骨性关节炎(KOA)的临床疗效。方法:将126例KOA患者随机分为治疗组和对照组,每组各63例,所有患者均给予关节镜下清理术,治疗组在此基础上加用活血消肿汤辨证治疗,连用15天。观察两组患者的临床疗效、症状体征改善情况及膝关节功能恢复情况。结果:治疗组总有效率为92.1%,显著高于对照组79.4%(P<0.05);治疗后,治疗组晨僵时间、关节压痛指数、关节肿胀指数及关节功能指数改善程度显著优于对照组(P<0.05);治疗后,两组活动范围评分、步行距离评分显著增加,但治疗组显著高于对照组(P<0.05)。结论:关节镜清理术联合活血消肿汤辨证治疗KOA安全有效,可明显缓解疼痛症状,改善膝关节功能,促进术后康复。
Objective： To explore clinical effect of arthroscopie debridement combined with Hexue Xiaozhong Decoction on treating osteoarthritis of the knee （KOA）. Methods： 126 eases of KOA patients were randomly divided into two groups. Con- trolled group （n = 52） was treated with arthroscopie debridement, and treatment group （n = 52） was treated with Hexue Xi- aozhong Decoction additionally for 15 days. The clinical effect, clinical symptoms and signs, recovery of joint function in 2 groups after treatment were observed and compared. Results： The total effective rate in treatment group and the controlled group respectively was 92. 1% and 74. 9%, the difference was statistically significant （ P 〈 0. 05 ）. The index of joint tenderness, swelling and function were significantly improved in the treatment group than those in the controlled group after treatment （P 〈 0. 05 ）. After treatment, the score of range of activity and walking distance in both groups were all increased, and the treatment group had more remarkable increase than that of the controlled group （ P 〈 0. 05 ）. Conclusions ： It is effective and safe to use Arthroseopic debridement combined with Hexue Xiaozhong Decoction to differentiate and treat KOA, it earl contribute to reduce the pain symptoms, improve knee joint function and accelerate the postoperative recovery.
