64排螺旋CT头颈联合CTA扫描方案的优化 被引量:4

摘要 目的探讨追踪触发技术在Philips 64排螺旋CT头颈联合CTA中扫描参数和对比剂剂量的最佳匹配方案。方法将60例患者分为2组。A组:对比剂剂量为体重(kg)乘以0.7 ml;B组(对照组):对比剂剂量为60 ml,2组的扫描参数均为:螺距0.891,球管旋转时间0.5 s,触发点设在主动脉弓层,阈值90 HU。结果 2组扫描方案得到的成像均能良好地、完整地显示颈动脉、椎动脉和颅内动脉。分别测量主动脉弓顶部、颈总动脉分叉下C5段、颈内动脉C1段、大脑中动脉四处动脉的CT值,A组的分别为:367 HU、418 HU、403 HU、350 HU,平均值为384 HU。B组的分别为:422 HU、498 HU、475 HU、406 HU,平均值为450 HU。A组的靶血管对比剂浓度比B组的更适合显示血管内斑块(适合显示血管内斑块对比剂浓度的CT值约为370 HU)。结论 64排螺旋CT进行头颈CTA检查时,应用追踪触发技术,选择对比剂剂量为体重(kg)乘以0.7 ml,对比剂注速5 ml/s,触发阈值90 HU即可获得满意的CTA图像,为临床诊断提供更为准确的依据。 Objective To investigate the best match program of scanning parameters and contrast agent dose in head and neck CTA for the tracking trigger technology in the Philips 64-shce spiral CT. Methods 60 patients were divided into two groups. For group A, the contrast agent dose was body weight { kg) multiplied by 0.7 ml;for group B ( control group), the con- trast agent dose was 60ml. Both groups'scanning parameters were:pitch 0.891, tube rotation time 0.5 s, the trigger point located at the aortic arch level, threshold 90HU. Results All the imagines from both scanning solutions completely display well carot- id artery, vertebral artery, and intracranial arteries. After measuring the CT values of the top of the aortic arch, the C5 segment of the carotid bifurcation,the C1 segment of the internal carotid artery and middle cerebral artery, group A displayed:367HU. 418HU,403HU 350HU, respectively, average 384HU. For group B, the values were respectively : 422 HU, 498HU, 475HU, 406HU, average 450HU. The contrast agent concentration of the target vessel in group A was more suitable than that in group B for vascular plaque ( the CT values of contrast agent concentration for plaque intravascular was about 370HU). Conclusion With employment of the tracking trigger technology,setting the contrast agent dose as body weight(kg) multiplied by 0.7 ml, the contrast agent injection speed as 5 ml/s and the threshold as 90HU in 64-slice spiral head and neck CTA can get satisfac- tory CTA images,which can provide more accurate basis for the clinical diagnosis.
