单排螺旋CT扫描技术在腕关节外伤中的应用研究 被引量:14

摘要 目的探讨单排螺旋CT扫描技术在腕关节外伤中的诊断价值。方法选择腕关节外伤78例,全部行X线平片及单排螺旋CT检查。CT分别行轴位、准冠状位及准矢状位扫描,观察不同扫描方式对腕骨骨折和关节关系的显示情况。结果 78例共骨折99处,9处不稳定或脱位。X线片检出85处骨折,不稳定或脱位7处。CT轴位扫描检出92处骨折,准冠状位扫描检出96处骨折,准矢状位扫描检出98处骨折,CT的3种描方式显示不稳定或脱位均为8处。结论单排螺旋CT检查应以X线平片为基础,针对病变部位,灵活选择扫描方式,扫描基线尽量垂直于骨折线。各种扫描方式相互补充,从多方位、多角度展示诸骨间关系,提高诊断的准确性。 Objective To explore the diagnostic value of spiral CT in application of traumatic wrist. Methods Seventy- eight patients with traumatic wrist underwent examinations of X-ray plain film and spiral CT. CT scanning included axial, coronal and sagittal positions. Imaging data was observed to detect the relationship between different scanning ways with wrist fracture and joints. Results Seventy-eight patients had ninety-nine fractures with nine astable or dislocations. X-ray film showed eighty-five fractures and seven astable or dislocations. Axial CT imaging showed ninety-two fractures. Coronal CT imaging showed ninety-six fractures. Sagittal CT imaging showed ninety-eight fractures. Eight astable or dislocations .were found by the three different sanning methods. Conclusion Spiral CT should be based on the X-ray plain film, and X-ray and spiral CT should be chosen according to diseased regions. Scanning baseline should be perpendicular to the fracture line as much as possible. These scanning methods com- plement each other, and demonstrate the relationship between the various bones to improve the diagnosis accuracy.
