采用低电压及低对比剂剂量行冠状动脉CT成像可行性研究 被引量:24

摘要 目的:探讨采用低管电压技术和低剂量对比剂(双低)进行64排螺旋CT冠状动脉检查的可行性。方法:采用GE公司宝石能谱CT Discovery CT750 HD,选取心率≤70次/min,节律规则的25例临床疑似冠心病患者,随机分为两组进行回顾性心电门控CTA检查,其中实验组11例,采用100kVp管电压,碘克沙醇(威视派克,270mg/ml)对比剂;对照组14例,采用120kVp管电压,碘普罗胺(优维显,370mg/ml)对比剂。对比分析两组冠状动脉图像CT值、图像质量、对比剂用量及辐射剂量。结果:实验组及对照组受检者均满足诊断要求,纳入统计冠状动脉节段分析显示,两组冠状动脉图像CT值差异有统计学意义;研究组平均有效辐射剂量为(8.97±1.78)mSv,对照组为(12.20±4.03)mSv,两组辐射剂量比较差异有统计学意义(P<0.05)。研究组对比剂用量16200mg,较对照组降低;而两组冠状动脉段图像质量比较差异无统计学意义(P>0.05)。结论:"双低"冠状动CTA成像有效降低对比剂用量和辐射剂量但图像质量不变,在心率较低的受检者中具有推广价值。 Purpose: To evaluate the feasibility of low tube voltage and low dosage contrast medium protocol in 64-slice CT coronary CT angiography (CCTA) by using retrospective electrocardiographic (ECG) triggering. Methods: Totally 25 patients with coronary disease underwent CCTA were included. All patients were randomly divided into experimental group (11 patients) with 100kVp and Iodixanol (270mgI/ ml), and control group (14 patients) with 120kVp and Iopromide (370mgI/ml). The following data, including image quality, CT value, contrast medium dosage and average radiation dose were analyzed between the 2 groups (p〉0.05). Results: No difference of the image quality were shown between these two groups. The CT value in experimental group was higher, and there was statistical difference between 2 groups on CT values (P〈0.05). The average radiation dose was (8.97±1.78) mSv in the experimental group and (12.20±4.03) roSy in the control group (p〈3.135). The contrast medium dosage in experimental group was 16200mgI. There was statistical difference of contrast medium dosage between 2 groups (p〈0.05). Conclusion: Low tube voltage and low dosage contrast medium can also achieve the result as the conventional protocol as well and can significantly reduce the radiation dose and contrast medium dosage.
