摘要
目的观察脑梗死患者轻度认知功能障碍(MCI)与血清总同型半胱氨酸(tHcy)、血脂水平之间的关系。方法测定50例脑梗死合并轻度认知功能障碍患者和50例年龄相匹配的正常对照者血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)及血清总同型半胱氨酸(tHcy)水平。结果 MCI组患者血清中TC、TG、HDL、LDL的水平分别为(5.72±1.25)mmol/L、(1.90±0.31)mmol/L、(1.03±0.12)mmol/L、(3.97±1.02)mm/L,而正常对照者相应指标分别为(4.27±1.04)mmol/L、(1.21±0.17)mmo/L、(1.34±0.25)mmol/L、(3.03±1.00)mmol/L,其中MCI组TC、LDL、TG均明显高于对照组,差异有统计学意义(P<0.05)。而HDL 2组比较差异无统计学意义(P>0.05)。MCI组血清tHcy水平明显高于对照组,差异有统计学意义(P<0.05)。血浆Hcy、血脂对预测MCI具有价值(P<0.05)。Logistic多因素回归分析提示,血浆Hcy、血脂是MCI的影响因素(OR值分别为1.11、1.56、1.07、1.25、3.39)。结论脑梗死MCI患者存在明显的脂质代谢紊乱及高同型半胱氨酸血症,Hcy、TC、LDL、TG可能对MCI的发病有一定影响,而HDL与MCI发病可能无关联。
Objective To investigate the correlation between total Homocysteine, blood lipid level and mild cognitive im- pairment(MCI) of stroke patients. Methods The serum levels of total cholesterol (TC), triglyceride (TG), high density lipo- protein (HDL), low density lipoprotein (LDL) and total homocysteine(tHcy) were measured in 206 MCI patients and 325 age- matched normal controls. Results The serum levels of TC, TG, HDL and LDL in MCI patients were (5.72 ± 1.25)mmol/L, (1.90±0.31)mmol/L, (1.03±0.12)mmol/L, (3.97±1.02)mm/L, while they were (4.27±1.04)mmol/L, (1.21±0.17) mmo/L, (1.34±0.25)mmol/L, (3.03 ± 1.00)mmol/L in controls. There was a significant difference in the serum levels of TC, LDL and TG between the two groups(P〉0.05), but not in the level of HDL between the two groups (P〉0.05). ROC curve showed tHcy level had some value of predicting the occurrence of mild cognitive impairment patients(P〈0.05). Logistic regression analysis showed that TC, TG, HDL, LDL and tHcy were independent risk factors for mild cognitive impairment patients(OR 1.11,1.56,1.07,1.25,3.39). Conclusion There is a metabolic disturbance of serum lipids, tHey in patients with MCI. they, TC, LDL and TG may increase the risk for the onset of MCI, while there may be no correlation of HDL with the onset of MCI.
