颈内动脉和大脑中动脉狭窄或闭塞脑梗死类型的比较 被引量:10

摘要 目的通过对颈内动脉(ICA)和大脑中动脉(MCA)主干动脉粥样硬化性狭窄或闭塞患者不同供血区脑梗死的对比研究,探讨其可能的发病机制。方法回顾性分析经磁共振弥散加权像(DWI)确诊的ICA/MCA主干粥样硬化性狭窄的急性脑梗死患者75例的临床资料和影像资料,依据患者发病1 w内头部磁共振弥散加权像(DWI)确定梗死部位并进行对比分析。结果确诊ICA 39例,MCA 36例;据头颅MRI表现,ICA组和MCA组分水岭梗死(BZI)分别为15/39例(38.5%)、22/36例(61.1%),MCA组显著高于ICA组(P<0.05);ICA组单发性脑梗死发生率明显高于MCA组(P<0.05);MCA组多发性脑梗死发生率显著高于ICA组(P<0.05)。结论颈内动脉和大脑中动脉狭窄或闭塞患者的脑梗死部位有显著性差异,前者以单发性脑梗死多见;后者以多发性脑梗死多见,多发性脑梗死中以皮层分支动脉供血区梗死(PI)+BZI多见。提示两者的发病机制可能存在不同之处。
