摘要
目的比较阿立哌唑与氯氮平治疗精神分裂症的效果及对糖脂代谢的影响。方法将2010年10月-2012年5月巴中市精神病防治院住院的精神分裂症患者101例分为阿立哌唑组(n=53)和氯氮平组(n=48),治疗时间为8周。在治疗前及治疗后2、4、6、8周末采用阳性与阴性症状量表(PANSS)评定疗效,血脂、空腹血糖和体重进行糖脂代谢评估。结果阿立哌唑组总有效率为92.45%,氯氮平组总有效率为95.83%,两组差异无统计学意义(x^2=0.932,P〉0.05)。两组整体改善精神症状的疗效相当,但氯氮平组在第2周末的PANSS总分及阳性症状分减少更明显(P〈0.05)。阿立哌唑组在第2.4、6、8周末的体重、血脂、血糖与治疗前比较差异均无统计学意义(P〉0.05)。氯氮平组的体重、三酰甘油在第6和8周末时高于治疗前,也高于阿立哌唑组(P〈0.05);空腹血糖在第4、6和8周末均明显高于治疗前,同时也高于阿立哌唑组(P〈0.05);而高密度脂蛋白、低密度脂蛋白、总胆固醇在组内和组间比较差异均无统计学意义(P〉0.05)。结论阿立哌唑和氯氮平疗效相当,但氯氮平起效更快;氯氮平对精神分裂症患者的糖脂代谢有显著影响,而阿立哌唑则影响较小。
Objective To compare the curative effects of Aripiprazole and Clozapine for patients with schizophrenia and their influence on glucose and lipid metabolism. Methods 101 patients with schizophrenia, admitted to Prophylactico-Therapeutic Hospital of Mental Illness of Bazhong City from October 2010 to May 2012 were assigned into Aripiprazole group (n = 53) and Clozapine group (n = 48) to receive treatment for 8 weeks. The curative effects were assessed using positive and negative symptoms scale (PANSS) at the end of 2, 4, 6, 8 week before and after treatment. The situation of glucose and lipid metabolism such as blood lipid, fasting blood sugar, and body weight were also evaluated. Results The overall effective rate of Aripiprazole group and Clozapine group was respectively 92.45% and 95.83%, with no significant between-group difference (X2=0.932, P 〉 0.05). The overall curative effects of improving mental symptoms in both groups were equivalency, but the Clozapine group had overall PANSS scores and positive symptoms scores reduced more significantly (P 〈 0.05) at the end of second week. The body weight, blood lipid, and blood glucose in Aripiprazole group at the end of 2, 4, 6, 8 week were not statistically different from those before treatment (P 〉 0.05). In Clozapine group, the body weight and triglycerides at the end of 6 and 8 week were higher than those before treatment, also higher than those in Aripiprazole group (P 〈 0.05); the fasting blood glucose at the end of 4, 6, and 8 week were significantly higher than those before treatment, also simultaneously higher than those in Aripiprazole group (P 〈 0.05). The high-density lipoprotein, low density lipoprotein, and total cholesterol had no within-group and between-group statistical differences (P 〉 0.05). Conclusion Aripiprazole and Clozapine has equivalent curative effects, but Clozapine works faster; Clozapine has significant influence on glucose and lipid metabolism of patients with schizophrenia, but Aripiprazole has less influence.
