摘要
目的分析儿童细菌性角膜炎病原菌构成,比较0.5%左氧氟沙星与0.3%氧氟沙星治疗儿童细菌性角膜炎的临床疗效及治愈时间的差异,以期为临床用药提供参考。方法选取2010年2月-2012年5月诊断为细菌性角膜炎的患儿160例,随机分成两组,每组各80例,前60例为主要研究对象,后20例为备选研究对象,分别给予0.5%左氧氟沙星(试验组)和0.3%氧氟沙星(对照组)滴眼液进行治疗并做细菌培养,观察两组的疗效、治疗时间和不良反应。结果儿童细菌性眼内炎以革兰阳性菌感染为主占79.6%,其中以表皮葡萄球菌居首位,两组分别占51.7%、56.7%,其次为金黄色葡萄球菌,两组分别占25.0%、26.7%;左氧氟沙星和氧氟沙星治疗儿童细菌性眼内炎的疗效差异无统计学意义;治疗7d内的治愈率氧氟沙星为43.3%、左氧氟沙星为63.3%,左氧氟沙星显著高于氧氟沙星治疗组(P=0.03);两组不良反应率分别为8.3%和10.0%,主要为眼部刺痛,差异无统计学意义。结论 0.5%左氧氟沙星滴眼液可以有效治疗儿童细菌性角膜炎,并且可以缩短治疗时间。
OBJECTIVE To observe the distribution of pathogens causing bacterial keratitis of children and compare the clinical efficacies of 0.5 % levofloxacin and 0.3 % ofloxacin eye drops in the treatment of bacterial keratitis and the healing time so as to guide the clinical use of antibiotics. METHODS A total of 160 children patients, who were diagnosed as the bacterial keratitis in the hospital from Feb 2010 to May 2012, were enrolled in the study and were randomly divided into two groups, with 80'cases in each; the first 60 cases were the major study objects and given the 0.5% levofloxacin (the study group), and the rest of 20 cases were the alternative study objects and treated with 0.3 % ofloxacin, then the bacterial culture was performed; the clinical efficacies, treatment time, and adverse effects were observed and compared between the two groups. RESULTS Of the pathogens causing the bacterial keratitis of children, the gram-positive cocci accounted for 79.6%, among which the Staphylococcus epiderrnidis was dominant, accounting for 51. 7% in the study group, 56. 7% in the control group, followed by the Staphylococcus aureus (26.0% in the study group, 26.7% in the control group). The difference in the clinical efficacy of treatment of bacterial keratitis between the levofloxacin and ofloxacin was not statistically significant. The cure rate of the treatment with ofioxacin within 7 days was 43.3%, the levofloxacin 63.3M ; the incidence of adverse reactions was 8.3% in the study group, 10.0% in the control group, the difference in the incidence of eye irritation or headache between the two groups was not statistically significant. CONCLUSION 0.5 % levofloxacin eye drops is an effective drdg for treatment of bacterial keratitis of children and can shorten the treatment time.
