ICU铜绿假单胞菌感染分布及耐药性分析 被引量:7

Distribution of Pseudomonas aeruginosa causing infection in ICU and analysis of drug resistance

摘要 目的对医院ICU分离的铜绿假单胞菌进行药敏分析,为ICU铜绿假单胞菌的耐药控制提供可靠依据。方法收集医院ICU 2009年1月-2011年12月分离的996株铜绿假单胞菌,采用VITEK-2Compact全自动药敏鉴定分析仪对菌株进行鉴定及药敏试验,WHONET5.3软件进行药敏结果分析。结果 2009-2011年ICU分离的铜绿假单胞菌主要来源为痰液,占85.5%~86.3%,尿液占4.9%~6.8%,分泌物占2.0%~3.7%,血液占0.9%~1.3%;2009-2011年对哌拉西林耐药率从42.0%升至75.9%、对哌拉西林/他唑巴坦耐药率从26.9%升至58.6%、头孢吡肟耐药率从26.5%升至47.9%、亚胺培南耐药率从48.9%升至76.9%、庆大霉素耐药率从13.2%升至71.8%、环丙沙星耐药率从40.6%升至80.5%、左氧氟沙星耐药率从54.3%升至79.1%;至2011年ICU分离菌株与非ICU分离的菌株相比对哌拉西林、哌拉西林/他唑巴坦、头孢吡肟、亚胺培南、阿米卡星、庆大霉素、环丙沙星、左氧氟沙星耐药率差异均有统计学意义(P<0.05)。结论 ICU分离的铜绿假单胞菌主要来源于痰标本,ICU分离菌株对多种抗菌药物耐药率显著高于非ICU菌株。 OBJECTIVE To analyze the drug susceptibility of Pseudomonas aeruginosa isolated from ICU so as to provide reliable basis for the control of drug resistance of P. aeruginosa isolated from the ICU. METHODS A total of 996 strains of P. aeruginosa isolated from the ICU from Jan 2009 to Dec 2011 were collected, the bacterial identification of isolates and drug susceptibility testing were performed by VITEK-2 Compact system, then the result of drug susceptibility testing was analyzed with the use of WHONETS. 3 software. RESULTS Of all the P. aeruginosa strains isolated from the ICU during 2009--2011, 85.5%-86.3% of the strains were isolated from the sputum, 4.9%-6.8% from the urine, 2.0%-3.7% from secretions, 0.9%-1.3% from blood. The result of drug susceptibility testing showed that during 2009-2011, the drug resistance rate of P. aeruginosa to piperacillin increased from 42. 0% to 75. 9%, piperacillin-tazobactam from 26. 9% to 58. 6%, cefepime from 26. 5% to 47.9M , imipenem from 48.9% to 76.9%, gentamicin from 13.2% to 71.8%, ciprofloxacin from 40. 6% to 80.5M, levofloxacin from 54. 3% to 79. 1%. As compared with the ICU isolates and the non-ICU isolates in 2011, the difference in the drug resistance rate to piperacillin, poperacillin-tazobactam, cefepime, imipenem, amikacin, gentamicin, ciprofloxacin, or levofloxacin was statistically significant (P〈0.05). CONCLUSION Most of the ICU isolates of P. aeruginosa are isolated from the sputum specimens, and the drug resistance rates of the ICU isolates to many antibiotics are significantly higher than those of the non-ICU isolates.
