切除骨桥并填塞带血供脂肪筋膜瓣治疗中心型骨桥形成的研究 被引量:4

Treatment of bone bridge formation by resection of bone bridge and filling of vascularized fat-fascia flap

摘要 目的 观察带血供脂肪筋膜瓣填塞对骨桥形成的治疗作用.方法 用3.5mm克氏针损伤兔一侧股骨远端中心骺板,另一侧作正常对照.实验动物分为4组,A组(骨桥切除+脂肪筋膜瓣填塞);B组(骨桥切除+游离脂肪填塞);C组为(单纯损伤);D组(单纯骨桥切除).20周后,对4组手术侧股骨长度、外翻角与健侧的差值进行比较,评价治疗效果.结果 运用筋膜脂肪瓣填塞骨桥切除后的缺损,可减轻股骨畸形的程度(P<0.01).A、B、C、D组的股骨短缩程度分别为(3.03 ±2.06)、(4.2±2.43)、(10.26 ±2.98)、(6.51 ±3.68) mm;外翻角的差异分别为(1.67±0.76).、(12.00±1.85)°、(23.47 ±3.02)°、(18.38±8.32)°.结论 填塞带血供脂肪筋膜瓣对骨桥形成有明显的治疗作用. Objective To investigate the therapeutic effectiveness of filling the injury with vascularized fat-fascia flap for bone bridge formation after epiphysea defect in juvenile rabbits.Methods The distal physeal femoral epiphyseal plate injury on one side in juvenile rabbits was made by a 3.5 mm Kirschner wire,and the opposite limb served as control.Experimental animals were divided into 4 groups:group A (resecting bone bridge and filling vascularized fat-fascia flap) ; group B (resecting bone bridge and filling free fat-fascia flap) ; group C (only injury without any fillings) ; group D (only resecting bone bridge).Twenty weeks later,the difference in the femur length and knee valgus angle in four groups between the operated side and the normal side was used to evaluate the degree of femoral deformity after epiphyseal plate injury.Results Filling of the vascularized fat-fascia flap into the defects following bone bridge resection could reduce the degree of femoral deformity (P 〈 0.01).The femur shorting length in groups A,B,C and D was (3.03 ±2.06),(4.2 ±2.43),(10.26 ±2.98),and (6.51 ±3.68) mm respectively,and the differences of valgus angles were 1.67 ± 0.76) °,(12.000 ± 1.85) °,(23.47 ± 3.02) °,and (18.38 ± 8.32) ° respectively.Conclusion Resecting bone bridge and filling of vascularized fat-fascia flap can treat bone bridge formation after epiphyseal injury,and reduce the deformities of femur after epiphyseal injury.
