摘要
目的对学龄前儿童补充维生素A(vitamin A,VA)、锌(Zinc,Zn),论证分析VA/Zn影响血红蛋白(hemoglobin,Hb)与红细胞生成素(erythropoietin,EPO)的反馈调节机制。方法在重庆市近郊7所规模〉200人的幼儿园中随机选取2所中的所有3~6岁符合纳入标准的232名学龄前儿童,按分层随机整群抽样的方法,以幼儿园为单位,分成单独补VA组(A组)和补VA加Zn组(AZ组)。A组与AZ组儿童又按不同VA与Hb基础水平分组,干预前贫血合并VA不足的儿童定义为Ⅰ组,Hb正常合并VA不足的为Ⅱ组,贫血合并VA正常的为Ⅲ组。营养素补充6个月,补充干预前后分别检测Hb、VA、Zn和EPO水平。结果营养素干预6个月后,血清VA水平AZ组显著升高0.05 μmol/L(P〈0.05),但A组无明显变化。A组和AZ组儿童血清Zn水平干预前后分别显著升高了2.95 μmol/L和3.67 μmol/L(P〈0.05, P〈0.01);Hb和EPO水平较干预前均显著升高,贫血的发生率均显著下降(P〈0.05, P〈0.01),但两组间变化值差异均没有统计学意义(P〉0.05)。A组Log10 EPO与Hb的直线回归方程斜率干预后显著下降了-0.009 9(P〈0.05),但AZ组无明显变化(P〉0.05)。Ⅰ组和Ⅲ组干预后Hb水平均显著升高(P〈0.05),其中Ⅲ组Hb水平升高较Ⅱ组有统计学差异(P〈0.05);Ⅰ组儿童Log10 EPO水平变化较其他2组更明显(P〈0.05)。结论VA调节EPO的生成能力可受机体Hb水平的影响,机体VA的营养水平影响Hb浓度对EPO的反馈性调节作用。
Objective A randomized trial of vitamin A/zinc supplementation in preschool children was conducted to analyze the impact of vitamin A (VA)/zinc on feedback regulatory effect of hemoglobin (Hb) on erythropoietin (EPO) level. Methods Two kindergartens were randomly selected out of 7 in a suburb of Chongqing, China. According to the method of stratified random cluster sampling in kindergartens, a total of 232 preschool children who met the eligibility criteria aged between 3-6 years were assigned randomly to 2 treatment groups: VA group (A group) and VA plus zinc group (AZ group). The groups were sub-grouped into subgroup Ⅰ (anemia and VA deficiency), subgroup Ⅱ (normal Hb and VA deficiency), and subgroup Ⅲ (anemia and normal VA). The supplementation lasted for 6 months. Hb, VA, EPO and Zn concentrations before and after the intervention were measured. Results After 6 months of supplementation, the VA concentration in AZ group was significantly increased by 0.05 μmol/L (P〈0.05), whereas it was not significantly increased in A group. A group and AZ group showed significantly improved zinc concentration of 2.95 μmol/L and 3.67 μmol/L (P〈0.05, P〈0.01), respectively. Hb and EPO showed significant increases in the 2 groups (P〈0.05). Moreover, the prevalence of anemia was decreased significantly in A group and AZ group. The slope of the regression line between Log10 EPO and Hb in A group was significantly decreased by -0.009 9 (P〈0.05), while it was not significantly changed in AZ group. There were significant increases of Hb concentration in subgroup Ⅰ and subgroup Ⅲ (P〈0.05), and Hb was increased more significantly in subgroup Ⅲ than in subgroup Ⅱ. EPO was increased more significantly in subgroup Ⅰ than in the other 2 subgroups. Conclusions VA level can influence the feedback regulatory effect of Hb on EPO level.
