血清碱性磷酸酶水平与动态血压及中心动脉压的关系 被引量:8

Association of serum alkaline phosphatase with ambulatory brachial and office central blood pressures

摘要 目的观察血清碱性磷酸酶(ALP)水平与动态血压及中心动脉压的关系。方法对2008-12-2011-12到上海交通大学医学院附属瑞金医院高血压科门诊就诊的怀疑有高血压但尚未进行降压治疗或以往曾服用过降压药但已停药2周以上的研究对象783名进行动态血压监测,利用SphygmoCor仪器测量中心动脉压,采用日立全自动生化仪7600-020测定血清ALP水平。结果简单相关分析显示,血清ALP水平与24h肱动脉收缩压及脉压、中心动脉收缩压及脉压均呈弱相关。多元逐步回归分析显示,血清ALP每增加2.72倍,外周肱动脉收缩压及脉压分别增加3.85(P=0.023)和3.04mm Hg(P=0.006),中心动脉收缩压及脉压分别增加6.20(P=0.002)和4.02mm Hg(P=0.002)。中心动脉收缩压和脉压增加幅度分别高于外周肱动脉收缩压和脉压,但差异无统计学意义(P>0.05)。结论血清ALP水平增加是肱动脉及中心动脉收缩压和脉压增高的独立危险因素。 Objective To investigate the association of alkaline phosphatase with ambulatory brachial and office cen-tral blood pressures. Methods Seven hundred eighty-three subjects suspected of having hypertension but not tak-ing antihypertensive drugs for at least 2 weeks were recruited in Hypertension Clinic of Ruijin Hospital from Decem-ber 2008 to December 2011. All subjects had 24 hour ambulatory brachial blood pressure measured. Central blood pressures were estimated by the SphygmoCor system in the clinic. Serum alkaline phosphatase was assayed by the automated Hitachi Bioanalyzer 7600-020. Results Linear correlation analyses showed that 24 hour brachial and of-fice central systolic blood pressure and pulse pressure weakly correlated with serum alkaline phosphatase. Multiple stepwise regression analyses confirmed that each 2. 72-time elevation of serum alkaline phosphatase was associated with an increase of 24-hour brachial systolic blood pressure and pulse pressure by 3.85(P=0. 023) and 3.04 mm Hg (P=0. 006), respectively, and central systolic blood pressure and pulse pressure by 6.20(P = 0. 002) and 4. 02 mm Hg(P=0. 002), respectively. The magnitude of the elevation in both systolic blood pressure and pulse pressure was higher at central than was at brachial artery, but these differences did not reach statistical significance (P〉0.05). Conclusion Elevated serum alkaline phosphatase is an independent risk factor for the increased bra-chial as well as central systolic blood pressure and pulse pressure.
