BECKMAN试剂检测妊娠各期特异性甲状腺功能指标正常值范围研究 被引量:13

Establishment of trimester-specific reference intervals for thyroid tests detected by BECKMAM assay

摘要 目的探讨BECKMAN试剂检测妊娠各期血清甲状腺功能指标的正常值范围。方法随机选取2012年12月至2013年2月重庆医科大学附属第一医院589名不同孕期正常孕妇及108名正常妇女,取其血清促甲状腺激素(TSH)、血清游离三碘甲状腺原氨酸(FL)、血清游离甲状腺素(FT。)检测值的中位数和双侧限值(第2.5、第97.5百分位数)建立正常值范围,并对各组激素水平进行比较,分析TSH、FT。之间相关性。结果(1)妊娠早、中、晚期TSH分别为1.42(0.13,3.99)、1.81(0.47,4.86)、1.91(0.58,5.51)mlU/L;FT。分另lJ为10.45(8.13~15.87)、8.13(5.68~10.58)、7.48(5.68~10.32)pmol/L。(2)除了TSH浓度在妊娠中期与晚期相比差异无统计学意义,TSH、FT2、FT3水平在妊娠各期之间差异均有统计学意义。与正常妇女相比,TSH在妊娠早期降低,从孕中期开始逐渐回升;FT。在妊娠早期增高,从孕中期开始逐渐降低;妊娠早期FT,无明显变化,孕中期开始逐渐降低。仅在妊娠早期,TSH与FT。之间存在相关性(P=0.000,r=-0.318)。结论BECKMAN试剂可作为检测妊娠各期甲状腺功能指标的特异性试剂。 Objective To establish the trimester-specific reference intervals for thyroid testing for pregnant women. Meth- ods 589 normal pregnant women and 118 normal non-pregnant women were recruited in our hospital. The 2. 5th and 97.5th percentiles were calculated as the reference intervals for dedication serum TSH, FT4 , FT3 levels during each trimes- ter. Results (1) In the first , second and third trimesters,TSH [M(P2.5-P975)] levelswere 1.42 (0.13,3.99),1.81 (0. 47,4. 86) ,and 1.91 (0. 58,5.51 ) mIU/L, respectively; FT4 [ M ( P2.5 - P97.5 ) ] levels were 10. 45 ( 8. 13 - 15.87 ), 8. 13(5.68 - 10. 58) and 7. 48(5. 68 - 10. 32) pmoL/L,respectively. (2)TSH level in the second trimester had no signifi- cant difference compared with that of the third trimester. There were statistical significant differences in TSH, FT, and FT3 levels in each trimester. Compared with normal healthy women, TSH level was decreased in the first trimester and began to rise in the second trimester. FT4 level was increased in the first trimester and then reduced gradually in the second trimes- ter. FT3 had no obvious change in the first trimester and reduced gradually in the second trimester. There was a correlation between TSH and FT4 ( P = 0. 000, r = - 0. 318 ) only in first trimester. Conclusion Marked changes in thyroid hormone levels exist between pregnant and non-pregnant women. Thyroid hormone levels also differ between different stages of gesta- tion. For this reason, it is necessary to establish trimester-specific reference intervals for thyroid tests. Our research has de- termined trimester-specific intervals for evaluating thyroid status using BECKMAM assay, which has filled the blank of tri- mester-specific intervals using such assay.
