MR弥散张量成像在早期帕金森病患者嗅觉障碍检测中的应用

Application of MR diffusion tensor imaging in detecting dysosmia of patients with early-stage Parkinson′ disease

摘要 目的探讨MR弥散张量成像(DTI)在早期帕金森病(PD)患者嗅觉障碍检测中的应用价值。方法以早期PD患者20例作为实验组,选择同期年龄、性别以及文化程度匹配的20例健康志愿者作为对照组,采用"五位嗅觉测试液"测定嗅觉察觉阈值(DT)和嗅觉辨别阈值(IT),计算各向异性分数(FA)和平均扩散图(MD)。结果实验组的DT、IT高于对照组(P<0.05),实验组嗅觉中枢的初级嗅皮质、直回和钩束的FA低于对照组(P<0.05),而两组嗅觉中枢的MD值无统计学差异(P>0.05)。结论 DTI是检测早期PD患者嗅觉障碍的敏感方法。 Objective To investigate the value of MR diffusion tensor imaging (MRDTI) in early Parkinson's disease(PD) patients with olfactory dysfunction. Methods The MRDTI was performed in 20 patients with early PD(group PD) and 20 healthy volunteers(group C). The odor- perceived threshold(DT) and olfactory discrimination threshold (IT) were measured by the five olfactory test liquid. Fractional anisotropy (FA) and mean diffusion figure (MD) were calculated by FDT. Results The DT and IT were significantly higher in group PD than those in group C (P〈0. 05). The FA of the primary olfactory cortex, gyrus rectus and uncinate fasciculus were significantly lower in group PD than those in group C(P〈0. 05), while MD of olfactory center was not significantly different between two groups(P〉0. 05). Conclusion MRDTI is a sensitive method to detect the olfactory dysfunction in the patients with early PD.
