摘要 目的对比研究一氧化碳(CO)中毒患者动脉血氧饱和度(SaO2)和脉搏血氧饱和度(SpO2)的差异。方法选择一氧化碳中毒患者78例,同时进行动脉血气分析检测和脉搏血氧饱和度监测,按血中碳氧血红蛋白含量多少把患者分为轻、中、重度中毒3组,比较各组患者动脉血气分析结果中SaO2和监护仪上SpO2的差异。结果轻度中毒组SaO295.83%±1.34%,中度中毒组SaO288.26%±2.72%,重度中毒组SaO281.42%±3.58%,轻度中毒组SpO296.11%±1.87%,中度中毒组SpO295.56%±1.85%,重度中毒组SpO292.77%±2.41%。轻度中毒患者两组间无统计学差异;中度、重度中毒患者SaO2、SpO2间有显著性差异。结论对于一氧化碳中毒患者脉搏血氧饱和度测定不能敏感反映患者乏氧情况,应结合血气分析中的动脉血氧饱和度、血中COHb测定等来判断机体乏氧程度,及早纠正缺氧,促进CO排出。 Objective To compare the difference of arterial oxygen saturation (SaO2 ) and pulse oxygen saturation (SpO2 ) in patients suffering from carbon monoxide (CO) poisoning. Methods Arterial gas analysis and pulse oximetry monitoring were carried out in 78 cases of carbon monoxide poisoning. The patients were divided into 3 groups: mild, moderate and se- vere poisoning according to blood carboxyhemoglobin levels. Level of SaO2 and value of pulse oximetry were analyzed. Results The levels of SaO2 in mild, moderate and severe poisoning patients were 95.83% ± 1.34%, 88.26% ± 2.72%, 81.42% ± 3.58%, respectively. The values of SpO2 in mild, moderate and severe were 96. 11% ± 1.87%, 94.26% ± 2.22% , 92.77% ±2. 41%, respectively. There was no statistical significant difference of SaO2,SpO2 in patients with mild poisoning but were significant in moderate, severe poisoning patients. Conclusion For patients with carbon monoxide poisoning, pulse oximetry is not sensitive toreflect the hypoxic conditions. It should be combined with SaO2 of blood gas analysis, blood COHb and other body measurement to determine the degree of hypoxia, as soon as possible to correct hypoxia and promote the discharge of CO.
