摘要
目的探索医院的组织因素对实施电子病历系统的影响。方法研究者以实习生的身份观察该医院实施电子病历系统的过程,对关键人物进行访谈,采用主题框架法对访谈资料进行分析。结果如果电子病历系统对医院实现其组织目标有利,则其实施会比较顺利;低年资医生由于压力、时间、习惯等问题对使用电子病历产生抵抗;高年资医生对电子病历系统较为支持;技术代理人对电子病历系统的实施起重要作用。民族和文化因素也影响电子病历的实施。结论卫生部门应该对不同级别的医院分别提出明确的要求,医院应该加强技术代理人的权威,应重视使用者对电子病历实施的影响,采用多种方式调动医护人员使用电子病历系统的积极性。
Objectives: To investigate the influence of organizational factors on the implementation of an Electronic Medical Record (EMR) system in a western hospital. Methods: Using participatory observation and in-depth interview to collect the information during the implementation of EMR. Thematic Framework Analysis is employed to analyze the data. Results: If the introduction of an EMR were in accordance with the development goals of a hospital, the implementation process would be smooth. Junior doctors would resist the EMR because of extra work pressure, time constraint and habit, while the seniors are more likely to support the system. Technical agents of EMR play an important role during the implementation. Ethical and cultural factors also influence the process. Conclusion: Health administration department should issue standard and requirement for different level hospitals. The hospital should strengthen the technical agents authority and users' acceptance of EHR.
