摘要
目的:探讨胎膜早破孕妇外周血、脐血和胎膜组织中氧化型α1-抗胰蛋白酶(ox-AAT)的表达变化与组织学绒毛膜羊膜炎(histological chorioamnionitis,HCA)的关系。方法:选择胎膜早破孕妇90例,根据是否合并HCA分为实验组和对照组。采用ELISA法检测孕妇外周血及新生儿脐血中ox-AAT的水平;逆转录-PCR技术检测胎膜组织中ox-AAT mRNA的表达水平。结果:病理结果示:实验组38例,对照组52例。实验组血清、脐血中ox-AAT水平(2.415±0.105)ng/L、(3.023±0.186)ng/L高于对照组(1.526±0.10)ng/L、(2.357±0.104)ng/L(P<0.05);实验组胎膜组织中ox-AAT mRNA(0.883±0.027)水平高于对照组(0.449±0.011)(P<0.05)。实验组血清、脐血及胎膜组织中ox-AAT的水平均成正相关(P<0.05)。实验组新生儿肺炎发病率高于对照组(P<0.05)。结论:胎膜早破孕妇外周血、脐血和胎膜组织中ox-AAT水平升高与合并HCA及新生儿肺炎相关。
Objective To investigate the expressions of oxidized alpha-I antitrypsin (ox-AAT) in maternal serum, newborn umbilical cord blood and fetal membrane in pregnant women with premature rupture of membranes (PROM) and their association with histological chorioamnionitis (HCA). Methods 90 pregnant women with PROM were divided into 2 groups according to the placental pathological examination: patients with HCA (experimental group, n = 38) and patients without HCA (control group, n = 52). The levels of ox-AAT in maternal serum and newborn umbilical cord blood were detected by ELISA, and that of ox-AAT mRNA in fetal membrane was detected by reverse transcriptase polymerase chain reaction (RT-PCR). Results The levels of ox-AAT in maternal serum and umbilical cord blood in experimental group were significantly higher than those in control group [ (2.415 ± 0.105)ng/L vs. (1.526±0.10)rig/L, (3.023 ± 0.186)ng/L vs. (2.357 ±0.104)ng/L, P〈 0.05]. Level of ox-AAT mRNA in fetal membrane in experimental group was significantly higher than that in control group [0.883 ±0.027 vs. 0.449 ±0.011 ,P 〈 0.05). The levels of ox-AAT in maternal serum, umbilical cord blood and fetal membrane were positively correlated with each other (P 〈 0.05). The morbidity of neonatal pneumonia in experimental group was higher than that in control group (P 〈 0.05). Conclusion The elevated levels of ox-AAT in maternal serum, umbilical cord blood and fetal membrane in pregnant women with PROM were closely related to HCA and neonate
