经阴道实时三维超声造影评价输卵管通畅性的应用研究 被引量:11

Application of Real-Time Three-Dimensional Hysterosalpingo Contrast-Sonography in the Diagnosis of Fallopian Tube Patency

摘要 目的:将经阴道实时三维输卵管造影(3D-HyCoSy)与X线子宫输卵管碘油造影(HSG)及腹腔镜(LSC)通染液诊断结果行对比分析,探讨其在诊断输卵管通畅性方面的价值。方法:选取在我院住院的37例不孕症患者,均拟行LSC手术以检查输卵管通畅性。术前均予常规3D-HyCoSy检查,其中26例术前根据自愿原则行HSG检查。通过敏感度、特异度、阳性及阴性预测值、总符合率的比较以评估3D-HyCoSy对输卵管通畅性的诊断价值。结果:以LSC检查结果为参照,3D-HyCoSy的诊断敏感度、特异度、阳性预测值、阴性预测值、诊断符合率分别为87%、83%、81%、89%及85%。其中26例以HSG检查为参照,3D-HyCoSy的诊断敏感度、特异度、阳性预测值、阴性预测值、符合率分别为90%、87%、82%、93%及88%,两组比较均无显著性差异(P>0.05)。结论:3D-HyCoSy可直观、清晰、全面地显示输卵管空间走形及评价通畅性,在诊断输卵管通畅性方面具有一定的价值。 Objective: To explore the diagnostic value of real-time three-dimensional hysterosalpingo- contrast-sonography (3D-HyCoSy) in the assessment of fallopian tube patency by comparing 3D- HyCoSy with X-ray hysterosalpingography (HSG) and laparoscopy (LSC). Methods: A total of 37 infertility patients in our hospital underwent 3D-HyCoSy examination before LSC. Among them, 26 patients also underwent X-ray HSG before LSC. The sensitivity, specificity, negative predictive value, and positive predictive value were compared to explore the diagnostic value of 3D-HyCoSy in the assessment of fallopian tube patency. Results: By the criteria of final LSC re- sults, the sensitivity, specificity, negative predictive value and positive predictive value and accu- racy rate of 3D-HyCoSywere87%, 83%, 81%, 89%, and 85%. By the criteria of X-ray HSG, the sensitivity, specificity, negative predictive value and positive predictive value and accuracyrate of 3D-HyCoSy were 90%, 87G, 82%, 93%, and 88%. There were no significant differ- ences between 3D-HyCoSy and HSG/LSC results (P〈0.05). Conclusion.. 3D-HyCoSy is better in space image display and evaluation of the fallopian tube patency. This technique is superior to X-ray HSG, becomes a useful diagnostic technique, and plays a therapeutic effect to some extent.
