3D腹腔镜与传统腹腔镜技术治疗肾上腺肿瘤的疗效比较 被引量:24

摘要 目的比较3D腹腔镜与传统腹腔镜技术在肾上腺肿瘤手术的疗效。方法我院于2012年12月至2013年8月应用腹腔镜手术治疗肾上腺肿瘤55例。对行3D腹腔镜肾上腺手术患者30例(30组)与同期行传统2D腹腔镜肾上腺手术患者25例(2D组)的临床资料进行比较。结果55例手术全部成功,手术时间55~125min,其中3D腹腔镜组(60±19)min,2D腹腔镜组(68±24)min,两组差别具有统计学意义(P〈0.05);术中出血10—150ml,其中3D腹腔镜组(25±14)ml,2D腹腔镜组(34±17)ml,两组差异具有统计学意义(P〈0.05),所有患者均未输血。55例患者术后住院3-9d,其中3D腹腔镜组(4.5±2.6)d,2D腹腔镜组(4.7±2.5)d,差异无统计学意义(P〉0.05)。两组患者均未出现严重并发症。住院费用(19654±807.3)元,3D腹腔镜组(19789±786)元,2D腹腔镜组(19593±823)元,差异无统计学意义(P〉0.05)。结论3D腹腔镜技术较传统2D腹腔镜技术在空间定位及深度感觉上有明显优势,一定程度上降低手术难度,缩短手术时间,同时不增加患者的经济负担,是一项值得推广的新技术。 Objective To compare the effect of 3D laparoscopic and traditional laparoscopic techniques for adrenal neoplasm. Methods From December 2012 to August 2013, a total of 55 cases of adrenel neoplasm were treated by laparoscopy. The clinical effect of 30 patients operated by 3D laparoscopic adrenaleetomy (3D laparoscopic group) and 25 patients by 2D laparoscopic adrenalectomy (2D laparoscopic group) were compared. Results All the operations were completed successfully. The surgery time ranged from 55 to 125 min, the time of 3D laparoscopic group was (60±19) min, and 2D laparoscopic group (68± 24) min, with significant difference (P 〈0.05). Intraoperative blood loss was 10±150 ml, of which 3D laparoscopic group (25±14) ml, 2D laparoscopic group (34±17) ml, with significant difference (P 〈 0.05). No patient need blood transfusion. The postoperative hospital time was 3 ~ 9 d, 3D laparoscopic group (4.5±2.6) d, 2D laparoscopic group (4.7±2.5) d, without significant difference (P〉0.05). No serious complications occurred. Hospitalization charge was (19654±807) yuan, 3D laparoscopic group of (19789±786) yuan, 2D laparoscopic group (19593±823) yuan. The two groups had no significant difference (P〉0.05). Conclusions Three-dimensional laparoscopic technology compared with traditional two-dimensional laparoscopie technology has obvious advantage in spatial location and the sense of depth. To some extent, 3D laparoscopy reduces the operation difficulty, shortens the operation time, at the same time does not increase the economic burden of patients. This new technology is worth promoting.
