磁共振弥散张量成像联合神经纤维束示踪技术在急性脑梗死中的临床应用 被引量:6

摘要 目的观察缺血性脑卒中急性期患者皮质脊髓束的状况,分析运动功能与皮质脊髓束受损情况的关系。方法临床诊断急性脑梗死并经常规磁共振成像和弥散加权成像检查证实为单侧脑梗死患者需行磁共振弥散张量成像(DTI)检查,共21例。重建FA图及三维纤维束示踪成像(DTT)图,测定脑梗死区及健侧相应部位脑组织的各项异性FA值、DCavg值,同时测量双侧大脑脚的FA、DCavg值。运用DTT显示通过病灶的纤维束特征。结果急性脑梗死中心区与对侧FA值分别为(0.18±0.08)、(0.36±0.14),DCavg值分别为(4.95±1.14)×10-3mm2/s、(8.41±1.21)×10-3mm2/s,急性期锥体束受累患者双侧大脑脚水平锥体束FA值、DCavg值差异无统计学意义,病变侧CST表现为受压、移位、变形和中断,DTI可清晰显示梗死灶与白质纤维束的空间位置关系及其受累程度,皮质脊髓束的损伤程度与临床神经功能症状相关。结论 FA值与皮质脊髓束成像联合应用可无创显示脑梗死皮质脊髓束的损伤,有助于判断运动功能受损情况及预测预后。 [Objective] To observe the condition of cortical spinal tract (CST) in acute cerebral ischemic stroke patients and analyze relationship between motor function and the impairment degree of CST. [Meth- ods] Patients with acute cerebral infarction were studied with conventional MRI, DTI and DTr to measure the values of fractional anisotropy (FA) and DCavg in the infarction area and the corresponding area. FA and DCavg were also measured in bilateral cerebral peduncles. Visualization of the fibertract was achieved by DTF. [Results] The FA values of the infarcted region and corresponding contralateral area were 0.18 ± 0.08 and 0.36 ± 0.14 in the acute stage (P〈0.05), the DCavg values were 4.95 ± 1.14 and 8.41 ± 1.21 (x10-3 mm2/ s) respectively. The FA and DCavg values of bilateral cerebral peduncles were not have statistical differences in acute cerebral infarction (P〉0.05). Lesions side as the compressive deformation performance CST, and shift and interrupt. CST damage degree was positively correlated with the degree of clinical neurological symptoms. [Conclusion] FA combined with DTr can noninvasively evaluate the acute cerebral infarction brain damage, it provides the evidence for the recovery of motor function and prognosis.
