摘要
目的探讨血清抗突变型瓜氨酸波形蛋白(MCV)抗体、抗环瓜氨酸肽(CCP)抗体、类风湿因子(RF)、超敏C-反应蛋白(hs-CRP)和全血红细胞沉降率(血沉,ESR)在类风湿关节炎(RA)早期诊断中的临床价值。方法对98例RA患者、88例疾病对照组和56例健康对照组分别采用酶联免疫吸附试验(ELISA)方法检测血清抗MCV抗体、抗CCP抗体浓度;采用免疫散射比浊法测定血清hs-CRP、RF的浓度;用ALIFAX TEST1全自动血沉仪检测ESR,并进行分析比较。结果 RA患者抗MCV抗体、抗CCP抗体、RF、hs-CRP和ESR均明显高于疾病对照组和健康对照组(P<0.01)。抗MCV抗体具有较高的敏感性和特异性(90.8%,93.8%)。五项指标联合检测的特异性最高,达到99.3%,其敏感性为61.2%。结论抗MCV抗体在RA的诊断中具有较高的敏感性和特异性,更有利于RA的早期诊断。五项指标联合检测的特异性最高,可进一步提高RA早期诊断的准确性。
Objective To explore the clinical value of anti-mutated citrullinated vimentin(MCV) antibodies,anti-cyclic citrullinated peptide(CCP) antibodies,rheumatoid factor(RF),high-sensitivity C-reactive protein(hs-CRP) and erythrocyte sedimentation rate(ESR) in early diagnosis of rheumatoid arthritis(RA).Methods The levels of serum anti-MCV and anti-CCP in 98 RA patients,88 cases in disease control group and 56 healthy controls were detected by ELISA.The levels of serum hs-CRP and RF in the same individuals were detected by immune nephelometry and ESR was measured by full-automatic erythrocyte sedimentation analyzer.The differences in anti-MCV,anti-CCP,hs-CRP,RF and ESR among patients with RA,disease control group and healthy controls were compared.Results The levels of anti-MCV,anti-CCP,RF,hs-CRP and ESR in RA patients were significantly higher than those of other patients and healthy controls(P 0.01).Anti-MCV has a high sensitivity and specificity(90.8%,93.8%).The sensitivity of combined detection of anti-MCV,anti-CCP,RF,hs-CRP and ESR in RA diagnosis was 61.2%,and the highest specificity was 99.3%.Conclusion Anti-MCV,which has a high sensitivity and specificity,is more conducive to the early diagnosis of RA.Combined detection of anti-MCV,anti-CCP,RF,hs-CRP ESR in serum is more helpful for increasing the accuracy of the early diagnosis of RA.
