摘要
目的探讨银杏叶提取物注射液对重症急性胰腺炎患者炎症介质的影响。方法将重症急性胰腺炎患者147例按照治疗方法分为对照组(71例)和观察组(76例),对照组患者给予常规治疗,观察组患者在对照组患者治疗基础上给予静脉注射银杏叶提取物注射液70 mg,连续治疗15 d后对2组治疗有效率、腹痛消失时间、血清淀粉酶转正时间及首次排便时间进行评定,并分析治疗前后2组患者外周血清中炎症介质肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)和白细胞介素1β(IL-1β)水平,以及外周单个核细胞Toll样受体4(TLR4)和CD14的表达水平。结果观察组患者治疗有效率为92.11%,明显高于对照组的78.87%,差异有统计学意义(P<0.05)。观察组患者腹痛消失时间、血清淀粉酶转正时间及首次排便时间均较对照组明显缩短,差异有统计学意义(P<0.05)。对照组和观察组患者治疗后血清中TNF-α、IL-6和IL-1β水平较治疗前下降,且观察组患者治疗后血清中TNF-α、IL-6和IL-1β水平较对照组下降更为明显,差异均有统计学意义(P<0.05)。对照组和观察组患者治疗后外周单个核细胞TLR4和CD14的表达水平较治疗前明显下降,且观察组下降水平更明显,差异均有统计学意义(P<0.05)。结论银杏叶提取物注射液可显著降低重症急性胰腺炎患者炎症介质和外周单个核细胞TLR4和CD14的表达水平。
Objective To explore the effect of extract of Ginkgo Biloba Leaves injection on inflammatory mediator in patients with severe acute pancreatitis. Methods One hundred and forty-seven patients with severe acute panereatitis were di- vided into control group(71 cases) and observation group(76 cases) according to treatment ways. Patients in the control group were treated with conventional therapy. The patients in observation group were treated with extract of Ginkgo Biloba Leaves in- jection 70 mg on the basis of control group. The treatment efficiency, extinction time of stomachache, the time of serum amylase turned normal,the first time of defecation,the expressions of tumor necrosis factor(TNF-α) ,interleukin-6(IL-6) and interleu- kin-1 β (IL-1β) in serum and the Toll-liked receptor 4 (TLR4) and CD14 expression in peripheral blood mononuclear cell were analyzed on the 15th day after treatment. Results The treatment efficiency in observation group(92.11% ) was significantly higher than that in control group (78.87%) ( P 〈 0.05 ). The extinction time of stomachache, the time of scrum amylase turned normal and the first time of defecation in observation group were significantly shorter than those in control group( P 〈 0.05 ). Compared with before treatment, TNF-α, IL-6 and IL-1β levels in observation group and control group significantly reduced af- ter treatment( P 〈0. 05 ). TNF-α, IL-6 and IL-1 βlevel in observation group after treatment reduced more than the control group( P 〈 0.05 ). TLR4 and CD14 expression of peripheral blood mononuclear cell in control and observation group after treat- ment significantly reduced than that before treatment( P 〈 0.05 ). TLR4 and CD14 expression of peripheral blood mononuclear cell in observation group significantly decreased than that in control group after treatment ( P 〈 0.05 ). Conclusion Extract of Ginkgo Biloba Leaves injection is worth to clinical application for reducing inflammatory mediator and TLR4 and CD14 expres- sion of peripheral blood mononuclear cell.
