摘要
目的 :明确心钠素和降钙素基因相关肽能否作为冠心病早期心功能障碍的预测指标。方法 :随机入选经冠状动脉造影者 6 0例 ,依据病变累及冠状动脉的支数分为 4个亚组 (0支、1支、2支、3支病变组 )各 15例 ;左心导管及左室电影造影测定血流动力学参数 ;放射免疫法测定血浆心钠素 (ANP)、降钙素基因相关肽(CGRP)浓度。结果 :左室射血分数随着冠状动脉病变程度加重逐渐降低 ,但亚组间差异不显著 ;舒张功能参数亚组间差异显著 ,以时间常数为明显 (P<0 .0 1) ,它同冠状动脉狭窄积分呈正相关 (r=0 .72 ,P<0 .0 1)。 ANP同冠状动脉狭窄积分、时间常数呈正相关 (r=0 .5 0和 0 .5 7,P均 <0 .0 1) ,同左室射血分数呈负相关 (r=- 0 .33,P<0 .0 5 )。 CGRP同冠状动脉狭窄积分、时间常数呈正相关 (r=0 .38和 0 .37,P<0 .0 5和 P<0 .0 1) ,同左室射血分数呈负相关 (r=- 0 .46 ,P<0 .0 1)。结论 :冠心病舒张功能障碍发生先于收缩功能障碍 。
Objective: To verify the prognostic value of plasma calcitonin generelated peptide (CGRP) level and arterial natriuretic peptide in early cardiac dysfunction in patients with coronary artery disease.Methods:Sixty patients were included in the study and divided into four groups according to the number of diseased coronary artery branches (0,1,2,3 diseased branch group, n =15 in each group) determined by coronary arteriography.Hemodynamic parameters were monitored with a left cardiac catheter and left ventricular cineangiography.Plasma ANP and CGRP concentrations were assayed by radioimmunoassay.Results:Left ventricular ejection fraction(LVEF) decreased as the number of diseased coronary artery branches increased,but there were no significant differences between subgroups.There was significant difference in diastolic function parameters between subgroups.Plasma ANP level was positively correlated to stenosis score and time constant ( r =0 50 and r =0 57,both P <0 01) and negatively correlated to LVEF ( r =-0 33, P <0 05).Plasma CGRP level was positively correlated to stenosis score and time constant ( r =0 38 and r =0 37, P <0 05 and P <0 01),and negatively correlated to LVEF ( r =-0 46, P <0 01).Conclusions:Coronary artery disease impairs diastolic function prior to systolic function;CGRP and ANP are important indexes for evaluating early cardiac dysfunction in patients with coronary artery disease.
