后颅窝及三叉神经体积与三叉神经痛之间的相关性研究 被引量:2

The MRI Volumetry of Posterior Fossa and Trigeminal Nerve in Patients with Trigeminal Neuralgia

摘要 目的探讨后颅窝及三叉神经体积与三叉神经痛之间的相关性,阐明原发性三叉神经痛的可能发病机制。方法 26例原发性三叉神经痛患者(病例组)和20名健康志愿者(对照组)入选本研究,采用3D-MRI容量分析法分别测量上述样本的后颅窝体积及双侧三叉神经体积,统计分析两组样本的后颅窝体积、患侧及健侧三叉神经体积差异。结果病例组及对照组的后颅窝体积分别为(142.9±14.5)cm3、(161.7±15.1)cm3,t检验显示二者之间差异有统计学意义(t=2.106,P<0.05);患侧和健侧的三叉神经体积分别为(0.057±0.032)cm3、(0.068±0.018)cm3,二者之间差异亦有统计学意义(t=2.3263,P<0.05)。但对照组双侧三叉神经体积的比较差异无统计学意义(t=1.2753,P>0.05)。结论后颅窝体积及三叉神经体积的缩小与原发性三叉神经痛之间可能存在一定的相关性,测量后颅窝及三叉神经的体积可以为临床解释三叉神经痛提供一定的依据。 Objective To determine whether the volume of posterior fossa and trigeminal nerve might represent a pre disposition factor for neurovascular compression in trigeminal neuralgia (TN). Methods Twenty six patients with unilat eral idiopathic TN and 20 healthy controls underwent MR/examination. The volume of posterior fossa and the trigeminal nerve on the TN side and non affected side were calculated using a standardized method. Results The posterior fossa vol ume in the TN group and control group were( 142.9 ±14.5 )em3 and (161.7± 15.1 )cm3. A significantly smaller volume of the posterior fossa was found in TN group(t =2. 106,P 〈0.05). The volume of trigeminal nerve on TN side and non af fected side were (0.057 ± 0. 032)em3 and (0. 068 ± 0. 018 )cm3, a significantly smaller volume of trigeminal nerve was found on the affected side (t =2. 3263 ,P 〈0.05). There was no significant difference of trigeminal nerve volume between two sides in control group ( t = 1. 2753, P 〉 0.05 ). Conclusion A smaller posterior fossa and trigeminal nerve may be correlated with the occurrence of a neurovaseular compression, and these findings support the neurovascular compression theory in idiopathic TN.
