摘要
目的 探讨经颅彩色三维多谱勒超声(3D-TCD)观察脑梗塞患者脑动脉血流状态的价值。方法 对139例经头颅磁共振(MRI)检查确诊的脑梗塞患者做3D-TCD检查,观察主要脑动脉的平均峰流速(Vm);脉动指数(PI)和频谱形态,并与61例元脑梗塞者作对比分析。结果 脑梗塞组各脑动脉Vm减低发生率比无脑梗塞组有增加趋势,在大脑中动脉、大脑前动脉、基底动脉显著增加(P<0.05);PI值增高伴谱态异常发生率较无脑梗塞组有增加趋势,在大脑前动脉、椎动脉显著增加(P<0.05)。结论 3D-TCD是观察脑梗塞病人脑动脉血流状态的有价值的检查方法,是筛选缺血性中风高危病人的重要手段。
Objective To investigate the valuation of three - dimensional transcranial color doppler ultrasonography (3D - TCD) on the changes of cerebral blood flow in patients with cerebral infaretion .Methods 3D- TCD was performed in 139 old cerebral infarction patients confirmed by cranial magnetic resonance imaging (MRI) and in 61 without cerebral infarction old subjects. Mean flow velocity(Vm), pulsatility index (PI) and the shape of frequency band of main cerebral arteries were observed and compared between the two groups .Results In cerebral infaretion group, the incidence of Vm decrease of all cerebral arteries was increased, and the incidences of Vm decrease of middle cerebral artery (MCA), anterior cerebral artery (ACA) and basilar artery (BA) were significantly increased in compare with control group, (P < 0.05); the incidence of PI increase and anomaly of shape of frequency band were increased, and ACA and vertebral artery (VA) were significantly increased (P < 0 .05). Conclusions Cerebral infarction patients usually have changes of cerebral blood flow. 3D - TCD is a valuable method of examination.
