摘要
目的评价联合检测降钙素原(PCT)和超敏C反应蛋白(hs-CRP)对上呼吸道感染的早期诊断价值。方法选取103例上呼吸道感染患者(上呼吸道感染组)、60例非感染患者(非感染组)和20例健康人(健康对照组),分别采用电化学发光法和散射免疫比浊法测定其血清中PCT和hs-CRP的含量,同时做痰培养。结果上呼吸道感染组患者的血清PCT和hs-CRP水平均明显高于非感染组和健康对照组(P<0.01)。非感染组患者的血清hs-CRP水平高于健康对照组(P<0.05),但其PCT水平比较,差异无统计学意义(P>0.05)。分别以PCT≥0.5 ng/ml和hs-CRP≥10 mg/L为阳性标准,诊断上呼吸道感染的灵敏度为73.79%和65.04%,特异度为69.90%和67.96%,而联合两项指标检测诊断上呼吸道感染的灵敏度则是92.23%,特异度为86.41%。结论联合检测PCT和hs-CRP有助于上呼吸道感染的早期临床诊断,可提高其诊断的灵敏度和特异度。
Objective To evaluate the diagnositic value of upper respiratory tract infection diagnosed early by joint detection of procalcitonin (PCT)and high- sensitive C-reactive protein (hs-CRP). Methods 103 patients with upper respira- tory tract infection (the upper respiratory tract infection grouop),60 non infected patients (the non infected group) and 20 cases of healthy people (the healthy control group) were selected,serum level of PCT and hs-CRP were determined by electrochemilumineseence method and immune nephelometry assay respectively.For all patients,sputum specimens were cultivated as a control. Results Serum level of PCT and hs-CRP in the upper respiratory tract infection group were significantly higher that in the the non infected group and the healthy control group (P〈0.01).Serum level of hs-CRP in the the non infected group were significantly higher than that in the healthy control group (P〈0.05),but there was no statistical difference of PCT in the two groups (P〉0.05).PCT≥0.5 ng/ml and hs-CRP≥ 10 mg/L were selected as the standard respectively,sensitivity of PCT and hs-CRP diagnosing upper respiratory tract infection were 73.79% and 65.04% respectively, and specificity of PCT and hs-CRP were 69.90% and 67.96% respectively,the sensitivity and specificity of joint detection of PCT and hs-CRP were 92.23% and 86.41% respectively. Conclusion Joint detection of PCT and hs- CRP are helpful to early diagnosis of upper respiratory tract infection,and it can improve sensitivity and specificity of diagnosed by PCT and hs-CRP.
