高压电弧致角膜损伤35例临床观察 被引量:3

Clinical observation of 35 cases with cornea damage caused by high-voltage electric arc

摘要 目的探讨高压电弧致角膜损伤的特点及诊治方法。方法收集2010年9月至2012年12月在北京积水潭医院眼科由高压电弧致角膜损伤患者,通过裂隙灯及角膜荧光素染色检查,按1982年全国眼外伤职业眼病学组通过的标准对角膜烧伤程度分度。分析其临床表现和治疗效果及预防措施。结果共收集35例(70只眼)。其中角膜烧伤Ⅰ度16只眼(23%),Ⅱ度42只眼(60%),Ⅲ度12只眼(17%),无Ⅳ度伤眼。角膜上皮剥脱面积基本与基质烧伤面积一致:“+”10只眼(14%),“++”30只眼(43%),“+++”30只眼(43%)。经促进角膜上皮修复及抗感染等对症支持治疗后,68只眼(97%)角膜完全愈合,67只眼(96%)视力恢复至受伤前水平。结论高压电弧致角膜损伤及时诊断,合理治疗可以取得较好预后。 Objective To analyze the clinical characteristics and to explore the diagnosis and treatment measures for the cornea damage caused by high-voltage electric arc. Methods Patients with cornea damage caused by high-voltage electric arc were collected from Sep 2010 to Dec 2012. The diagnosis was based on the slit lamp and cornea fluorescein staining examination and the cornea burns degree grading was based on the grading standards formulated by the national ocular trauma and occupational eye disease association in 1982. The clinical features and efficacy were retro-spectively analyzed and the treatment and preventive measures were summarized. Results Thirty-five cases with 70 eyes were collected. The cornea bums degree grading was as follows: 16 eyes (23%) of degree I, 42 eyes (60%) of degree 1I, 12 (17%) eyes of degree llI and no one of degree IV. The cornea epithelial exfoliation area grading was as follows: 10 eyes (14%) of "+", 30 eyes (43%) of "++", 30 eyes (43%) of "+++". The cornea of 68 eyes (97%) were completely healed, the visual acuity of 67 eyes (96%) recovered to the pre-injury level. Conclusions Cornea damage caused by high-voltage electric arc can receive satisfactory prognosis with proper and prompt treatment.
