摘要
目的观察穴位磁热治疗糖尿病周围神经病变的临床疗效及对糖尿病足神经性溃疡的预防作用。方法选取确诊为糖尿病周围神经病变的住院患者80例,随机分为治疗组和对照组各40例。治疗组用糖尿病治疗仪对患者的中脘、命门、三焦俞、太溪、复溜、双侧足三里穴和三阴交穴进行磁热治疗,2次/d,每次20 min;同时给予甲钴胺500μg,1次/d,肌肉注射。对照组予甲钴胺500μg,1次/d,肌肉注射。2周为一疗程,间隔3个月开展下一疗程治疗,共治疗3个疗程,观察两组患者治疗前、后症状,震动感觉阈值,周围运动和感觉神经传导速度的变化及足部溃疡的发生情况,分析穴位磁热治疗糖尿病患者周围神经病变的疗效。结果治疗后,两组患者症状均较前改善;运动和感觉神经传导速度均较治疗前明显提高,且治疗组显著优于对照组(P<0.05);治疗组患者震动感觉阈值较治疗前明显降低(P<0.05);治疗组患者足部神经性溃疡发病率明显低于对照组。结论临床联合穴位磁热治疗糖尿病周围神经病变有效,并可能预防糖尿病足神经性溃疡的发生。
【Objective】To investigate the effects of acupoints magnetic heat treatment on diabetic peripheral neu- ropathy, and on preventing neuropathic ulcers in diabetic foot.【Methods】80 patients with diabetic peripheral neu- ropathy(DPN) were randomly divided into treatment group(n =40) and control group(n =40). The treatment group was treated with acupoint magnetic heat(20 min, bid) combined with Methylcobalamin(500 μg, i.m., qd) and the con- trol group was treated with Methylcobalamin(500 μg, i.m., qd) alone. The patients were treated 2 weeks for a course, with an interval of 3 months to carry out the next course. The total therapy involved 3 courses. Before and after treat- ment, the symptoms, vibration perception threshold(VPT), peripheral motor nerve and sensory nerve conductive ve- locities, foot ulceration were observed. The effects of combined treatment on DPN were assayed. 【Results】After treatment, the improvement of symptoms was observed in both groups. A significant increase in motor and sensory nerve conduction velocity was observed in both groups, and more obvious in treatment group(P 0.05). An obvious decrease of the VPT was observed in treatment group(P 0.05). The incidence of diabetic neuropathic foot ulceration in treatment group was significantly lower than that in control.【Conclusion】Acupoints magnetic heat therapy is an effective therapy in DPN, and may be a possible way for preventing diabetic neuropathic foot ulceration.
