椎间盘源性腰痛的病理生理改变与诊疗技术的进展 被引量:3

摘要 椎间盘源性腰痛在慢性下腰痛中常见。免疫组织化学研究发现椎间盘源性腰痛患者出现椎间盘的解剖结构紊乱,原本并无神经血管支配的纤维环内2/3区及髓核组织中发现有神经及血管组织,同时在退变的椎间盘中由于细胞外基质的合成与降解失衡,致椎间盘基质丢失,其完整性受到破坏,可缩性下降,导致椎间盘在生物应力作用下出现纤维环的撕裂,而纤维环的撕裂致椎间盘细胞释放大量的炎性细胞因子,这可能与腰痛的机制息息相关;另一种观点认为腰痛的来源很可能是软骨终板的损伤引起的。椎间盘源性腰痛的诊断标准一直争议不断,椎间盘造影一直以来被认为是椎间盘源性腰痛诊断的金标准,但是也有学者提出了质疑,磁共振(MRI)影像学检查中高信号区(HIZ)与Modic改变对诊断椎间盘源性腰痛提供很大的帮助。文章综述了椎间盘源性腰痛的病理生理改变与诊疗技术的进展。
