摘要
目的研究有晶状体眼后房型人工晶状体又称可植入式接触镜(implantable contact lens,ICL)植入术治疗成人近视性屈光参差性弱视的效果和安全性。方法2010年9月到2012年9月在汉口爱尔眼科医院行ICL植入术的伴有弱视的高度近视性屈光参差患者共11例,随访6个月以上。记录患者术前、术后裸眼视力、最佳矫正视力、眼压、角膜内皮细胞计数以及术后的Titmus立体视情况。结果术前患者弱视眼平均屈光度为13.55 D(等效球镜),术后为0.33 D。术前最佳矫正视力为0.50±0.18,术后1周裸眼视力为0.72±0.23,末次随访时裸眼视力进一步提升到0.78±0.22,较术前最佳矫正视力平均提高2行,差异有显著统计学意义(t=-9.87,P<0.001)。术前角膜内皮细胞计数为(2678±155)个·mm-2,术后1周为(2466±214)个·mm-2,末次随访时为(2410±142)个·mm-2。术后Titmus立体视检查2例斜视患者没有立体视,其他患者均有立体视,立体视为157″±102″。所有术眼术中、术后均未见严重并发症发生。结论ICL植入术是治疗成人近视性屈光参差性弱视安全有效的方法,其能明显提高患者的视功能,改善患者的生活质量。
Objeetive To study the efficacy and safety of phakic posterior cham ber intraocular lens (implantable contact lens, ICL) implantation for adult myopic ani sometropic amblyopia. Methods From September 2010 to September 2012 in Hankou Aier Eye Hospital, a total of 11 patients with the high myopic anisometropic amblyopia were implanted ICL and followed up for 5 months or more. The preoperative and post operative uncorrected visual acuity, best corrected visual acuity, intraocular pressure, corneal endothelial cell count and postoperative Titmus stereopsis were recorded. Re suits The preoperative and postoperative average refractive error of amblyopia eyes were 13.55 D (spherical equivalent) and 0.33 D. The mean preoperative best corrected visual acuity was 0.50 ± 0.18, the postoperative uncorrected visual acuity at 1 week was 0.72 ±0.23 ,further increased to 0.78 ±0.22 at the last follow-up time,which increased 2 lines compared with preoperative best corrected visual acuity, and there was significant difference (t = -9.87 ,P 〈 0.001 ). The preoperative corneal endothelial cell count was (2578 + 155 ) cells . mm-2,the postoperative one week was (2455 ±214) cells . mm-2 ,the last follow-up time was (2410 ± 142) cells . mm-2. The postop erative stereoscopic tests by Titmus, there were two strabismus patients without stereopsis, and other patients had stereop- sis of 157"± 102". No obviously intraoperative or postoperative complication was observed. Conclusion Posterior cham ber phakic intraocular lens implantation is safe and effective for adult myopic anisometropic amblyopia, which can obviously improve the visual function and quality of life.
