摘要
目的:探讨杏仁核三维重建技术对立体定向手术治疗难治性精神分裂症患者术后多巴胺(DA)代谢的影响。方法:我院收治的60例难治性精神分裂症患者随机分为对照组(杏仁核MRI二维图像指导手术)和研究组(杏仁核三维重建图像指导手术),测定分析两组手术治疗前后精神分裂症患者脑脊液(CSF)中DA和高香草酸(HVA)的变化情况。结果:两组患者术后DA及HVA显著下降(P<0.05),研究组手术前后DA及HVA下降水平明显高于对照组,其差异具有统计学意义(P<0.05)。结论:杏仁核三维重建技术运用于立体定向手术有助于纠正难治性精神分裂症患者中枢DA代谢的紊乱,为治疗难治性精神分裂症患者提供了新的研究方向。
Objective To investigate the influence of amygdala three dimensional (3D) reconstruction technique on the change of dopamine (DA) metabolism in treatment-resistant schizophrenia patients after operation. Methods 60 schigophrenia patients in our hospital were randomized into control group (surgery under the guidance of amygdala MRI 2D imaging) and observation group (surgery under the guidance of amygdala 3D reconstruction imaging). The changes of dopamine (DA) and homovanillie acid (HVA) in eerebrospinal fluid (CSF) before and after surgery in the two groups were measured and analyzed. Results The levels of DA and HVA in CSF decreased significantly after the operation in both groups (P 〈 0.05), especially in observation group, the decrease was more significant (P 〈 0.05). Conclusions The amygdala 3D reconstruction technique used in stereotaetie operation can help correct the disorder of DA metabolism in central nervous system in refractory schizophrenia patients, and provide a new research direction for the treatment for refractory schizophrenia.
