分化型甲状腺癌大剂量^(131)I治疗后双侧腮腺肿胀患者腮腺功能的变化研究 被引量:2

Parotid Functional Changes in Patients with Bilateral Parotid Swelling after Large Doses of ^(131)I Therapy for Differentiated Thyroid Carcinoma

摘要 目的分析分化型甲状腺癌(DTC)131I治疗后双侧腮腺肿胀患者腮腺摄取和分泌排泄功能的变化。方法对21例服131I清除DTC转移灶治疗后双侧腮腺肿胀的乳头状甲状腺癌患者行腮腺核素动态显像,观察其治疗情况和治疗前后双侧腮腺摄取功能和分泌排泄功能的改变。结果 131I治疗后1~2周患者两侧腮腺出现不同程度肿胀、疼痛,肿胀后1周内行核素动态显像腮腺时间-放射性曲线上升幅度降低。治疗后双侧腮腺摄取指数和酸刺激排泄指数均较治疗前降低,差异有统计学意义(P<0.05)。结论 131I治疗后DTC腮腺肿胀患者腮腺摄取、分泌排泄功能均受损,其中分泌排泄功能受损更显著,应引起临床重视。 Objective To evaluate the changes of parotid functions of uptake, secretion and excretion in patients with bilateral parotid swelling before and after radioactive iodine -131 (131 I) treatment for differentiated thyroid carcinoma (DTC). Methods A total of 21 patients with papillary thyroid cancer, whose bilateral parotid glands were swelled after radioactive 13~I treatment for DTC metastases, were included in this study. Dynamic imaging of the parotid glands was performed, and the parot- id functions of uptake, secretion and excretion before and after treatment were collected. Results In all 21 cases, bilateral pa- rotid glands became swelled with pain after one to two weeks of radioactive 1311 treatment. The dynamic imaging in one week after parotid swelling showed that the rise of time vs. radioactivity curve became slower. The bilateral parotid functions of uptake, se- cretion and excretion became lower after the treatment (P 〈 0.05 ). Conclusion Parotid functions of uptake, secretion and ex- cretion of DTC patient with bilateral parotid swelling after TM I therapy are damaged, in which the damage to its secretory and ex- cretory function is more severe, which should be paid attentionduring clinical process.
